摘要
目的:探讨急诊心电图对高钾血症的快速评价作用。方法:选择2020年1月~2022年6月急诊收治的疑似高钾血症患者84例为对象,采用急诊心电图为患者检查,将血钾浓度检测结果作为金标准,研究急诊心电图对高钾血症的快速评价作用。结果:急诊心电图检测灵敏度为97.5%,特异度为75.0%,准确度为96.43%。血钾浓度3.4~5.5mmol/L心电图异常率为25.0%,5.6~6.5mmol/L心电图异常率为94.74%,6.6~7.5mmol/L心电图异常率为94.74%,>7.5mmol/L心电图异常率为100.0%。高钾血症患者心电图主要为帐篷状高尖T波,部分为T波尖但不高或T波高但不尖。结论:高钾血症患者采用急诊心电图快速评价的作用显著,有较高的灵敏度与特异度、准确度,血钾浓度越高的患者心电图异常率越高。
Objective:To investigate the rapid evaluation effect of emergency electrocardiogram on hyperkalemia.Methods:84 suspected hyperkalemia patients admitted to emergency department from January 2020 to June 2022 were selected as subjects.Emergency electrocardiogram was used as patient examination,and the blood potassium concentration detection result was used as the gold standard to study the rapid evaluation effect of emergency electrocardiogram on hyperkalemia.Results:The sensitivity,specificity and accuracy of emergency ECG were 97.5%,75.0%and 96.43%.Blood potassium concentration 3.4-5.5mmol/L ECG abnormal rate 25.0%,5.6-6.5mmol/L ECG abnormal rate 94.74%,6.6-7.5mmol/L ECG abnormal rate 94.74%,>7.5mmol/L ECG abnormal rate 100.0%.The electrocardiogram of hyperkalemia patients was mainly tent-shaped high-pointed T-wave,some were T-wave pointed but not high or T-wave high but not sharp.Conclusion:Rapid evaluation of emergency electrocardiogram in patients with hyperkalemia has a significant effect,with high sensitivity,specificity and accuracy.The higher the concentration of blood potassium,the higher the abnormal rate of electrocardiogram.
