摘要
目的:分析在老年患者APFN术中对患者采取超声引导下腰丛-坐骨神经阻滞治疗的效果。方法:选择复旦大学附属金山医院麻醉科2020年1月~2021年12月收治的APFN术老年患者50例为观察对象,分为观察组(超声引导下腰丛-坐骨神经联合阻滞)和对照组(常规硬膜外穿刺置管干预),对比分析两组患者治疗后感觉运动神经阻滞起效和持续时间、镇痛持续时间、血流动力学水平变化、麻醉效果、不良反应发生率。结果:观察两组之间的麻醉效果,可见两组间的麻醉效果对比差异明显(P<0.05);血流动力学分析,观察组血流动力学相对稳定,对照组更高(P<0.05);两组不良反应发生率分析,观察组患者相对更低,两组间对比差异明显(P<0.05);观察两组之间感觉、运动神经阻滞起效时间、感觉、运动神经阻滞、镇痛持续时间,可见观察组时间更长,对照组更短(P<0.05)。结论:在老年APFN术患者的临床治疗中对患者采取超声引导下腰丛-坐骨神经联合阻滞治疗可明显提高患者的麻醉效果,同时可有效缩短患者神经阻滞起效时间,延长麻醉药物作用时间,并有利于术中患者血流动力学稳定,副作用更小,安全性更大,阻滞效果更好,能够减轻应激反应,术后并发症发生率明显更低。
Objective:To analyze the effect of ultrasound-guided low back plexus-sciatic nerve block in elderly patients with APFN.Methods:A total of 50 elderly patients receiving treatment in our hospital from January 2020 to December 2021 who need to be treated with APFN were selected as the observation objects.The patients were divided into observation group and control group on average according to random number table method.The effect and duration of sensorimotor nerve block,duration of analgesia,changes of hemodynamic level,anesthetic effect and incidence of adverse reactions were compared and analyzed between the two groups after treatment.Results:After observing the anesthesia effect between the two groups,the two groups were significantly(P<0.05);the hemodynamic analysis was relatively stable in the control group(P<0.05);The incidence of adverse reactions in the two groups(P<0.05);The onset time of sensory,motor nerve block,sensory,motor nerve block and analgesia duration between the observation group was longer,and the control group was shorter(P<0.05).Conclusion:In the clinical treatment of elderly APFN surgery for patients with low back plexus-sciatic nerve block guided by ultrasound treatment can obviously improve the effect of anesthesia in patients with,at the same time can effectively shorten the nervous tissue in patients with working time,extend the time of the anesthetic effect,and can be beneficial to patients with intraoperative hemodynamic stabilization and reduce stress reaction,significantly lower incidence of postoperative complications.
