摘要
目的:分析宫外孕腹腔镜手术患者麻醉需求,对比分析气管插管麻醉、喉罩全麻对患者血流动力学与安全性的影响。方法:选择2019年3月~2020年6月本院收治的90例宫外孕腹腔镜手术患者,进行随机法分组麻醉,伦理委员会审核批准。对照组(n=45)宫外孕患者行气管插管麻醉,观察组(n=45)宫外孕患者行喉罩全麻,比较2组患者不同时间点心率(HR)、平均动脉压(MAP)变化情况、麻醉恢复时间、拔管时间以及腹胀、呛咳等并发症发生情况。结果:观察组组内各个时间点HR、MAP比较,差异不明显(P>0.05);对照组组内部分时间点HR、MAP比较,差异显著(P<0.05);组间部分时间HR、MAP比较,观察组对比对照组平稳(P<0.05);组间麻醉恢复时间、拔管时间比较,观察组患者麻醉恢复、拔管时间均明显短于对照组(P<0.05);组间腹胀、躁动、呛咳并发症发生情况比较,观察组发生率4.44%低于对照组的17.77%(P<0.05)。结论:喉罩全麻用于宫外孕腹腔镜手术中麻醉效果理想,患者血流动力学平稳,明显缩减了患者的麻醉恢复、拔管时间,提高了手术患者的安全性。
Objective:To analyze the anesthesia needs of patients undergoing laparoscopic surgery for ectopic pregnancy,and to compare and analyze the effects of tracheal intubation anesthesia and laryngeal mask general anesthesia on the hemodynamics and safety of patients.Methods:Select 90 patients undergoing laparoscopic surgery for ectopic pregnancy in our hospital from March 2019 to June 2020,and underwent randomized group anesthesia,and the ethics committee reviewed and approved them.The control group(n=45)patients with ectopic pregnancy underwent tracheal intubation anesthesia,and the observation group(n=45)patients with ectopic pregnancy underwent laryngeal mask general anesthesia.The heart rate(HR)and mean arterial pressure(mean)of the two groups were compared.Arterial pressure,MAP changes,anesthesia recovery time,extubation time,abdominal distension,coughing and other complications.Results:Comparison of HR and MAP at various time points in the observation group showed no significant difference(P>0.05);Comparison of HR and MAP at some time points in the control group showed significant differences(P<0.05);HR and MAP at part of the time between groups in comparison,the observation group was stable compared with the control group(P<0.05);Compared with the anesthesia recovery time and extubation time between the groups,the anesthesia recovery and extubation time of the observation group were significantly shorter than those in the control group(P<0.05);The abdominal distension and extubation between the groups comparing the incidence of restlessness and coughing complications,the incidence of 4.44%in the observation group was lower than 17.77%in the control group(P<0.05).Conclusion:General anesthesia with laryngeal mask used in laparoscopic surgery for ectopic pregnancy has an ideal anesthesia effect.The patient’s hemodynamics is stable,which significantly reduces the patient’s anesthesia recovery and extubation time,and improves the safety of surgical patients.
