摘要
目的:构建复合手术切除颅内动静脉畸形围术期护理敏感指标体系。方法:以“结构⁃过程⁃结果”三维质量评价模式为模型,通过2轮专家函询确立复合手术切除颅内动静脉畸形围术期护理敏感指标体系。结果:2轮函询专家的权威系数均为0.921,2轮函询专家的积极系数分别为93.33%和100.00%。2轮函询的结构、过程和结果指标的肯德尔和谐系数比较,差异均有统计学意义(P<0.001)。经过连续2轮的专家函询,最终形成的复合手术切除颅内动静脉畸形围术期护理敏感指标体系包括3个一级指标、8个二级指标、42个三级指标,所有指标的重要性均>4.0分、变异系数均<0.25。结论:构建的复合手术切除颅内动静脉畸形围术期护理敏感指标体系是一套科学、可靠、全面、系统的护理质量评价体系,为医院职能部门及管理者对专科医疗护理质量评价提供了依据,同时也为医疗机构专科医疗护理服务质量的优化和持续改进提供了参考。
Objective:To construct the perioperative nursing sensitivity index system for intracranial arteriovenous malformations resection with compound surgery.Methods:The perioperative nursing sensitivity index system for intracranial arteriovenous malformations resection with compound surgery was formed through 2 consecutive rounds of expert correspondence with the three⁃dimensional quality evaluation model of"structure⁃process⁃result".Results:The authority coefficient of experts in 2 rounds of correspondence was 0.921,and the positive coefficient of experts in 2 rounds of correspondence was 93.33%and 100.00%respectively.The Kendall W comparison of the structure,process and result indicators of the 2 rounds of correspondence showed significant differences(P<0.001).The perioperative nursing sensitivity index system for intracranial arteriovenous malformations resection with compound surgery eventually formed after 2 consecutive rounds of expert correspondence included 3 first level indicators,8 second level indicators and 42 third level indicators,the importance assignment of all indicators were greater than 4.0,and the coefficient of variation was smaller than 0.25.Conclusion:The perioperative nursing sensitivity index system for intracranial arteriovenous malformations resection with compound surgery constructed is a scientific,reliable,comprehensive and systematic nursing quality evaluation system,which provides a basis for hospital functional departments and managers to evaluate the quality of specialized medical care,and also provides a reference for the optimization and continuous improvement of specialized medical care service quality in medical institutions.
