摘要
目的探讨竖脊肌平面阻滞(ESPB)对老年胸腔镜肺癌根治术患者心电图的影响。方法60例择期行胸腔镜肺癌根治术的老年患者,随机分为实验组和对照组,各30例。实验组应用竖脊肌平面阻滞联合全身麻醉,对照组应用单纯全身麻醉联合静脉自控镇痛。比较两组患者手术前后心电图异常表现(心动过缓、心动过速、ST-T改变、早搏)。结果术前3 h,两组心动过缓、心动过速、ST-T改变、早搏占比比较差异无统计学意义(P>0.05);术中,实验组心动过速、ST-T改变、早搏占比分别为36.7%、36.7%、46.7%,均低于对照组的63.3%、63.3%、76.7%,差异具有统计学意义(P<0.05);术后3 h,实验组ST-T改变、早搏占比分别为40.0%、43.3%,均低于对照组的70.0%、73.3%,差异具有统计学意义(P<0.05)。结论对老年胸腔镜肺癌根治术患者应用竖脊肌平面阻滞可以降低心律失常及心肌缺血的发生率。
Objective To discuss the effect of erector spinae plane block(ESPB)on electrocardiogram in elderly patients undergoing thoracoscopic radical resection of lung cancer.Methods A total of 60 elderly patients undergoing thoracoscopic radical resection of lung cancer were randomly divided into experimental group and control group,with 30 cases in each group.The experimental group received erector spinae plane block combined with general anesthesia,and the control group received simple general anesthesia combined with patient controlled intravenous analgesia.The abnormal electrocardiogram manifestations(bradycardia,tachycardia,ST-T changes,premature beats)before and after surgery were compared between the two groups.Results 3 h before surgery,there were no statistically significant differences between the two groups in terms of bradycardia,tachycardia,ST-T changes and premature beats(P>0.05).During the surgery,the proportions of tachycardia,ST-T changes and premature beats were 36.7%,36.7%and 46.7%in the experimental group,which were lower than those of 63.3%,63.3%and 76.7%in the control group,and the differences were statistically significant(P<0.05).3 h after surgery,the proportions of ST-T changes and premature beats were 40.0%and 43.3%in the experimental group,which were lower than those of 70.0%and 73.3%in the control group,and the differences were statistically significant(P<0.05).Conclusion Erector spinae plane block can lower the incidence of arrhythmia and myocardial ischemia in elderly patients undergoing thoracoscopic radical resection of lung cancer.
