摘要
目的探讨慢性阻塞性肺疾病(COPD)急性加重期伴低氧血症患者血清肌钙蛋白、心肌酶水平变化及其临床意义。方法选取2020年1月-2022年1月湖北省黄石市第四医院接诊的慢性阻塞性肺疾病急性加重期(AECOPD)患者123例,将动脉氧分压(PaO2)值≥60mmHg患者62例纳入对照组(n=62),PaO2值<60mmHg的61例AECOPD患者纳入观察组(n=61);比较两组血清肌钙蛋白I(cTnI)、肌酸激酶(CK)和肌酸激酶同工酶(CK-MB)水平,比较不同低氧血症严重程度的AECOPD患者血清cTnI、CK、CK-MB水平,采用Spearman相关性分析法分析低氧血症严重程度与血清cTnI、CK、CK-MB的关系。结果观察组血清cTnI、CK、CK-MB水平显著高于对照组(P<0.05);不同低氧血症严重程度的AECOPD患者血清cTnI、CK、CK-MB比较,差异有统计学意义(P<0.05);其中中度、重度低氧血症患者cTnI、CK、CK-MB显著高于轻度组,重度低氧血症患者cTnI、CK、CK-MB显著高于中度组(P<0.05);Spearman相关性分析显示,低氧血症严重程度与血清cTnI、CK、CK-MB呈显著正相关关系(r=0.916、0.665、0.805,均P<0.05)。结论AECOPD伴低氧血症患者血清cTnI、CK、CK-MB水平高于单纯AECOPD患者,且血清cTnI、CK、CK-MB水平与低氧血症的严重程度正相关,值得临床重视。
Objective To investigate the changes and clinical significance of serum troponin and myocardial enzymes in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and hypoxemia.Methods A total of 123 patients with AECOPD admitted to the hospital from January 2020 to January 2022 were selected.Patients with arterial partial pressure of oxygen(PaO2)≥60mmHg were included in the control group(n=62),and patients with PaO2<60mmHg were included in the observation group(n=61).The levels of serum troponin I(cTnI),creatine kinase(CK)and creatine kinase isoenzyme(CK-MB)were compared between the groups,and between patients with different severity of hypoxemia.Spearman correlation analysis was performed to analyze the relationship between the severity of hypoxemia and serum cTnI,CK and CK-MB.Results The levels of serum cTnI,CK and CK-MB in the observation group were significantly higher than those in the control group(P<0.05).The levels of serum cTnI,CK and CK-MB increased with the aggravation of hypoxemia(P<0.05).Spearman correlation analysis showed that the severity of hypoxemia was significantly positively correlated with serum cTnI,CK and CK-MB(r=0.916,0.665,0.805,P<0.05).Conclusion The levels of serum cTnI,CK and CK-MB in patients with AECOPD and hypoxemia are higher than those in patients with simple AECOPD.The levels of serum cTnI,CK and CK-MB are positively correlated with the severity of hypoxemia,which deserves clinical attention.
