神经传导联合交感皮肤反应检测2型糖尿病患者神经病变的应用价值 被引量:1

摘要 目的 探讨神经传导联合交感皮肤反应检测2型糖尿病患者神经病变的应用价值。方法 选取2022年1月~12月台州市路桥区第二人民医院内科收治的2型糖尿病患者99例作为研究对象,按照不同的检测方法将其分为三组,其中有糖尿病周围神经病变(DPN)临床表现且神经电生理检查异常的患者归为DPN组(33例),无DPN临床表现而仅有电生理检查异常的患者归为亚临床DPN组(33例),无周围神经损害症状的患者归为NDPN组(33例);并且将神经电生理检查异常(即DPN组与亚临床DPN组)的患者归为糖尿病神经性病变患者(66例)。比较三组患者的神经传导速度、交感皮肤反应,评估神经传导速度联合交感皮肤反应检测在2型糖尿病神经病变患者诊断中的价值。结果 DPN组、亚临床DPN组患者运动神经传导及感觉神经传导速度均比NDPN组慢(P <0.05);DPN组和亚临床DPN组的上、下肢平均潜伏期与NDPN组相比均明显延长(P <0.05);DPN组和亚临床DPN组的平均波幅与NDPN组相比,均明显降低(P <0.05)。对于糖尿病患者神经病变的诊断,神经传导联合交感皮肤反应联合检测的检出率最高为96.96%。建立受试者工作(ROC)曲线,曲线结果显示神经传导速度检测、交感皮肤反应检测以及两者联合检测曲线下面积分别为0.795、0.636、0.826,灵敏度分别为77.95%、71.72%、89.48%,特异度分别为72.95%、75.58%、86.35%,两者联合诊断2型糖尿病神经病变具有良好的预测价值。结论 神经传导速度联合交感皮肤反应检测2型糖尿病患者神经病变的诊断价值较高,对患者感觉、运动及自主神经进行检测,为2型糖尿病周围神经病变的临床确诊提供客观指标支持。 Objective To explore the application value of nerve conduction combined with sympathetic skin response in the detection of patients with type 2 diabetic neuropathy.Methods Ninety-nine patients with type 2 diabetes admitted to the Department of Internal Medicine of the Second People's Hospital of Taizhou Luqiao District from January 2022 to December 2022 were selected as the study objects and divided into three groups according to different detection methods.Among them,patients with clinical manifestations of diabetic peripheral neuropathy(DPN) and abnormal neuroelectrophysiological examination were classified into DPN group(33 cases).Patients with no clinical manifestations of DPN and only abnormal electrophysiological examination were classified into subclinical DPN group(33 cases),and patients with no symptoms of peripheral nerve damage were classified into NDPN group(33 cases).Patients with abnormal neuroelectrophysiological examination(DPN group and subclinical DPN group) were classified as neuropathic patients(66 cases).Nerve conduction velocity and sympathetic skin response were compared among the three groups to evaluate the value of nerve conduction velocity combined with sympathetic skin response in diagnosis of type 2 diabetic neuropathy.Results The motor nerve conduction velocity and sensory nerve conduction velocity in DPN group and subclinical DPN group were slower than those in NDPN group(P<0.05).The average latency of upper and lower limbs in DPN group and subclinical DPN group was significantly longer than that in NDPN group(P<0.05).The mean amplitude of DPN group and subclinical DPN group was significantly lower than that of NDPN group(P<0.05).For the diagnosis of neuropathy in diabetic patients,the highest detection rate of nerve conduction combined with sympathetic skin response was 96.96%.Receiver operating(ROC) curve was established,and the curve results showed that the area under the curve of nerve conduction velocity detection,sympathetic skin response detection and combined detection were
