摘要
目的:探讨以患者为中心构建互联网+模式在老年慢性心力衰竭(CHF)患者中的应用效果。方法:选择本院收治的170例老年CHF患者为对象,以随机数字表法分为行常规干预组和互联网+组(在常规治疗基础上增加以患者为中心构建互联网+模式干预),各85例。连续干预3个月后,比较两组患者的心功能指标、遵医行为、负性情绪、生活质量、满意度等指标的变化。结果:3个月后,与常规干预组比较,互联网+组LVEF[(67.97±7.10)%比(73.10±8.48)%]、每搏输出量(SV)[(63.78±8.95)ml比(69.65±8.06)ml]及康复依从性、WHOQOL-100生活质量评分、总满意度(78.82%比92.94%)均显著提高,QT间期离散度(QTd)[(63.70±7.04)ms比(54.93±6.02)ms]、左室舒张末内径(LVEDd)[(53.10±6.81)mm比(46.03±5.65)mm]及负性情绪评分包括汉密尔顿焦虑量表14(HAMA-14)[(14.96±2.78)分比(10.09±2.77)分]、汉密尔顿抑郁量表17(HAMD-17)[(18.94±4.20)分比(13.04±3.56)分]评分均显著降低(P<0.05或<0.01)。结论:以患者为中心构建互联网+在老年CHF患者中的应用效果良好,能够提高患者的心功能,改善患者的负性情绪,提高患者的康复依从性、生活质量和满意度。
Objective:To explore the application effect of patient-centered "Internet +" construction in aged patients with chronic heart failure(CHF).Methods:A total of 170 aged CHF patients treated in our hospital were randomly and equally divided into routine intervention group and Internet+ group(received patient-centered Internet + model intervention based on routine treatment).After three-month intervention,cardiac function indexes,medical compliance behavior,negative emotion,quality of life and satisfaction etc.indexes were compared between two groups.Results:Compared with routine intervention group after three months,there were significant rise in LVEF[(67.97±7.10)%vs.(73.10±8.48)%],stroke volume(SV) [(63.78±8.95)ml vs.(69.65±8.06)ml],rehabilitation compliance,WHOQOL-100 quality of life score,total satisfaction rate(78.82%vs.92.94%),and significant reductions in QT interval dispersion(QTd) [(63.70±7.04)ms vs.(54.93±6.02)ms],left ventricular end-diastolic dimension(LVEDd) [(53.10±6.81)mm vs.(46.03±5.65)mm],scores of Hamilton anxiety scale 14(HAMA-14) [(14.96±2.78) scores vs.(10.09±2.77) scores]and Hamilton depression score 17(HAMD-17) [(18.94±4.20) scores vs.(13.04±3.56) scores]in Internet+ group,P0.05 or 0.01.Conclusion:Patient-centered "Internet +" construction possesses good application effect in aged CHF patients.It can improve patients' cardiac function,negative emotion,rehabilitation compliance,quality of life and satisfaction.
