摘要
目的 分析血清肿瘤标志物单独以及联合检测在原发性肝癌中的诊断价值。方法 回顾性选取2020年2月—2022年2月曹县人民医院疑似原发性肝癌患者50例的临床资料。运用化学发光酶免分析方法测定血清胆碱酯酶(cholinesterase, CHE)、γ-谷氨酰转肽酶(gamma-glutamyl transpeptidase, GGT)、α-L-岩藻糖苷酶(α-L-fucosidase, AFU)、甲胎蛋白(alpha-fetoprotein, AFP)、铁蛋白(ferritin, SF)等肿瘤标志物水平。统计分析肿瘤标志物单独与联合检测的阳性情况,并统计分析肿瘤标志物单独与联合检测的诊断效能。以病理学检查为金标准。结果 AFP、SF的灵敏性、准确性均高于CHE、GGT、AFU,差异有统计学意义(P<0.05)。联合检测灵敏性为100.00%,特异性为100.00%,准确性为100.00%,阳性预测值为100.00%,阴性预测值为100.00%,联合检测灵敏性、准确性均高于CHE、GGT、AFU、AFP、SF,差异有统计学意义(P<0.05),联合检测的阴性预测值高于AFP、SF,差异有统计学意义(P<0.05)。结论 血清肿瘤标志物水平在原发性肝癌诊断中的应用价值高,联合检测较单独检测的诊断效能高。
Objective To analyze the diagnostic value of serum tumor markers alone and combined detection in pri‐mary liver cancer.Methods Clinical data of 50 patients with suspected primary liver cancer in Caoxian People's Hos‐pital from February 2020 to February 2022 were retrospectively selected.Serum levels of cholinesterase(CHE),γ-glu‐tamyl transpeptidase(GGT),α-L-fucosidase(AFU),alpha-fetoprotein(AFP),ferritin(SF)and other tumor markers were determined by chemiluminescence enzyme immunoassay.The positive status of single and combined detection of tumor markers were statistically analyzed in the two groups,and the diagnostic efficacy of single and combined detec‐tion of tumor markers was statistically analyzed.Pathological examination was the gold standard.Results The sensitiv‐ity and accuracy of AFP and SF were higher than those of CHE,GGT,and AFU,and the difference was statistically significant(P<0.05).The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of the combined test were 100.00%,100.00%,100.00%,100.00%,and 100.00%,respectively.The sensitivity and accu‐racy of the combined test were higher than those of CHE,GGT,AFU,AFP,and SF,and the difference was statistically significant(P<0.05).The negative predictive value of the combined test was higher than that of AFP and SF,and the difference was statistically significant(P<0.05).Conclusion The level of serum tumor markers is of high value in the diagnosis of primary liver cancer,and the combined detection is more effective than the single detection.
