摘要
目的探讨等离子联合银质针在椎间盘源性腰痛(DLBP)合并腰背部筋膜炎治疗方面的临床效果。方法选择北京丰台右安门医院疼痛科2021年1月至2022年2月收治患有DLBP合并筋膜炎患者120例,随机数字表法分为等离子联合银质针组(联合银质针组)和等离子组,每组60例。等离子组予常规低温等离子髓核消融治疗,联合银质针组在等离子组治疗基础上加用银质针配合治疗。分别评估治疗后1及6个月时的临床效果,评估患者的疼痛状况、症状或体征改善状况。结果治疗后1及6个月时,联合银质针组患者显效率均高于等离子组(P<0.05),腰痛功能评价表(JOA)治疗后1个月和6个月均高于等离子组(P<0.05);视觉模拟评分(VAS)治疗后1个月和6个月均低于等离子组(P<0.05);生活质量评分和满意度与等离子组相比都相对较高(P<0.05)。结论在治疗DLBP合并筋膜炎随访1个月和6个月时两组均有效,而低温等离子髓核消融术与银质针联合的方案效果更好,机体疼痛等症状及体征改善显著,生活质量显著提高。
Objective To investigate the clinical effect of cryogenic plasma and silver needle in the treatment of disogenic low back pain(DLBP)with low back fasciitis.Methods One hundred and twenty patients with disogenic low back pain and low back fasciitis in Department of Painology,Beijing Fengtai Youanmen Hospital from January 2021 to February 2022 were selected,and randomly divided into group of cryogenic plasma combined with silver needle and group of cryogenic plasma,with 60 patients in each group.Group of cryogenic plasma received conventional hypoogenic plasma nucleus pulposus ablation,and group of plasma combined with silver needle added silver needle.The clinical effects were assessed at 1 and 6 months after treatment,and improvement in pain status,symptoms or signs.Results After 1 and 6 months treatment,the efficiency and JOA in group of plasma combined with silver needle was higher than that in group of cryogenic plasma(P<0.05),but VAS was lower in group of plasma combined with silver needle than in group of cryogenic plasma(P<0.05).Quality of life and satisfaction were higher in group of plasma combined with silver needle than that in group of cryogenic plasma(P<0.05).Conclusion The cryogenic plasma nucleus pulposus ablation combined with silver needle had better efficacy for the treatment of patients with DLBP and fasciitis,both the symptoms and signs and the quality of life improved significantly.
