摘要
目的 探讨品管圈(quality control circle,QCC)在住院医师放射科影像诊断报告质量持续改进中的价值。方法 随机选取2019年3月10名住院医师书写的1696份放射科影像诊断报告为对照组,应用品管圈后再随机选取这10名住院医师在2019年9月撰写的1761份放射科影像诊断报告为QCC组。比较对照组、QQC组住院医师放射科影像诊断报告总缺陷率、重点缺陷原因比率的差异。计算品管圈目标达标率及进步率。结果 QCC组住院医师放射科影像诊断报告总缺陷率低于对照组[3.92%(185/1696)比10.91%(69/1761)],差异有统计学意义(P<0.001)。QCC组影像诊断报告存在标点符号不规范、错别字、漏字、未描述病灶与周围结构关系、前后内容矛盾等重点缺陷原因的比率均低于对照组,差异有统计学意义(均P <0.05)。QCC组影像诊断报告使用专业术语不规范、“左右”部位描述错误的比率均低于对照组,但差异均无统计学意义(均P> 0.05)。品管圈目标达标率为105.59%,进步率为61.32%。结论 品管圈能有效改进住院医师放射科影像诊断报告质量,是放射科专业基地医疗质量管理的重要工具。
Objective The aim of the study was to analyse the value of the quality control circle(QCC) in continuously improving the imaging diagnostic report quality of radiology residents.Methods A total of 1696radiology reports from 10 residents in March 2019 were randomly selected as the control group.After applying the optimized process assisted by QCC,1761 imaging diagnostic reports from the same 10 residents in September 2019 were randomly selected to be the QCC group.The differences in the total defect rate and the proportion of key defects between the control group and QCC group were compared.The achievement rate and progress rate of the QCC were calculated separately.Results The total defect rate of imaging reports of radiology residents in the QCC group was lower than that in the control group [3.92%(185/1696) versus 10.91%(69/1761)],and the difference was statistically significant(P<0.001).Rates in major flaw causes of radiology reports including nonstandard punctuation,typos,missing characters,no description of the association between lesions and adjacent structures and inconsistency in the QCC group were fewer than those in the control group respectively.There were statistically significant differences between the two groups(all P<0.05).Incidence rates of nonstandard terminology and laterality errors in the QCC group were smaller than those in the control group,while statistical differences were not observed significantly(all P>0.05).The achievement rate and improvement rate were 105.59 % and 61.32 % respectively.Conclusion QCC can effectively improve the quality of imaging diagnostic reports written by radiology residents,which is an important quality control tool for standardized residency training in radiology bases.
