摘要
目的:探讨案例需求导向式护理模式在髋关节假体周围感染行二次翻修术患者中的应用效果。方法:将2018年1月1日~2020年1月31日45例髋关节假体周围感染行二次翻修术患者设为对照组,实施常规护理;将2020年2月1日~2022年1月31日45例髋关节假体周围感染行二次翻修术患者设为观察组,实施案例需求导向式护理模式。比较两组入院当日及出院前1 d行为疼痛评分量表(BPS)评分、症状自评量表(SCL-90)评分、Harris量表评分,并发症(深静脉血栓形成、肺栓塞、关节脱节)发生率。结果:两组出院前1 d BPS评分、SCL-90评分均低于入院当日(P<0.01),观察组出院前1 d BPS评分、SCL-90评分低于对照组(P<0.01);两组出院前1 d Harris量表评分均高于入院当日(P<0.05),观察组出院前1 d Harris量表评分高于对照组(P<0.05);观察组并发症发生率低于对照组(P<0.05)。结论:对髋关节假体周围感染行二次翻修术患者实施案例需求导向式干预模式,可减少并发症,减轻疼痛感,改善髋关节功能。
Objective:To explore the application effect of case-oriented intervention mode in nursing care of patients with hip prosthesis infection undergoing second-stage revision.Methods:A total of 45 patients with periprosthetic infection of hip joint who received the second-stage revision treatment from January 1,2018 to January 31,2020 were divided into the control group and received routine nursing,and another 45 cases from February 1,2020 to January 31,2022 were divided into the observation group and were given case-oriented intervention mode nursing care.On the day of admission and the day before discharge,the scores of Behavioral Pain Scale(BPS),symptom checklist 90(SCL-90),Harris Hip Score,and the incidence of complications(deep venous thrombosis and pulmonary thrombosis embolism and joint dislocation).Results:One day before discharge,the BPS scale scores and SCL-90 scores in both groups were lower than those on the day of admission(P<0.01),and those in the observation group were lower than those in the control group(P<0.01).The Harris scale scores in both groups were higher than those on the day of admission(P<0.05),and those in the observation group were higher than those in the control group(P<0.05).And the incidence of complications was lower in the observation group than that in the control group(P<0.05).Conclusion:The application of case-oriented intervention mode can minimize the complications of patients with hip prosthesis infection treated by two-stage revision,relieve their pain,and improve hip function.
