摘要
目的探讨皮肤透明细胞汗腺瘤的高频彩色多普勒超声特征,并结合病理进行分析。方法对31例经病理证实为透明细胞汗腺瘤病例的高频彩色多普勒超声图像进行回顾性分析。结果31例病例中,肿物位于头面部15例,下肢6例,颈部5例,手掌2例,腰部1例,胸壁2例。肿物均边界清晰,形态规则,且均为囊实性结节,其中21例以实性为主,10例以囊性为主,实性部分呈附壁结节;31例肿物实性部分均见血流信号,Adler血流分级表现为1~3级,其中1级6例、2级10例、3级15例。组织病理学特征性表现为真皮内界限清楚的囊实性结节,由位于中央的圆形空泡状透明细胞和位于周边的嗜碱性细胞两种肿瘤细胞构成。结论皮肤透明细胞汗腺瘤具有“囊中有实、实中有囊”的特征性高频超声表现,可为透明细胞汗腺瘤的临床诊断提供帮助。
Objective To explore the characteristics of high-frequency color Doppler ultrasound of clear cell hidradenoma(CCH)of the skin,and analyze it with pathology.Methods The high-frequency color Doppler ultrasound images of 31 cases with CCH confirmed by pathology were analyzed retrospectively.Results Among the 31 cases,the masses were located on the head and face in 15 cases,lower limbs in 6 cases,neck in 5 cases,palms in 2 cases,waist in 1case,and chest wall in 2 cases.The 31 cases of masses were all cystic solid nodules,and all had clear borders and regular shapes.21 cases were mainly solid,10 cases were mainly cystic,and the solid part showed mural nodules;The solid part of the 31 cases showed blood flow signals,and the Adler blood flow classification was grade 1-3,including 6 cases of grade 1,10 cases of grade 2,and 15 cases of grade 3.Histopathological characteristics are characterized by cystic solid nodules with clear boundaries in the dermis,which are composed of two types of tumor cells,a circular vacuolated clear cell located in the center and a basophilic cell located in the periphery.Conclusion The cystic component has solidity,and the solid component has cysticity,which is the characteristic high-frequency ultrasound manifestations of clear cell hidradenoma of the skin,which can provide help for the clinical diagnosis for CCH.
