摘要
目的探究脐孔筋膜平台入路在妇科单孔腹腔镜中的应用效果。方法选取2021年3月至2023年3月桐庐县第一人民医院收治的妇科良性疾病患者60例,依据患者意愿将其分为3组,其中接受脐部康基单孔Port入路平台单孔腹腔镜手术者20例(设为A组),接受脐孔筋膜入路平台单孔腹腔镜手术者20例(设为B组),接受多孔腹腔镜手术者20例(设为C组)。对比三组的临床相关指标、瘢痕情况、伤口愈合程度、并发症发生率。结果三组的手术时间、术后肛门排气时间、住院时间、术中出血量、伤口愈合程度及并发症发生率相比差异无统计学意义(P>0.05);B组、C组的手术费用相比差异无统计学意义(P>0.05);B组、C组的手术费用低于A组(P<0.05)。术后1个月,B组的切口美观满意度高于A组、C组(P<0.05);A组的切口美观满意度高于C组(P<0.05)。术后1 d,三组的疼痛评分相比差异无统计学意义(P>0.05);术后3 d,B组的疼痛评分低于A组、C组(P<0.05)。术后7 d,三组的温哥华瘢痕评价量表(VSS)评分相比差异无统计学意义(P>0.05);术后1个月,B组的VSS评分低于A组、C组(P<0.05);A组的VSS评分低于C组(P<0.05)。结论妇科单孔腹腔镜中应用不同入路方式均能够顺利完成LESS,其中经脐孔筋膜平台入路能够在不增加患者经济费用、保障治疗效果的基础上,减少瘢痕对外观影响,且术后疼痛程度低,患者的切口美观满意度明显高。
Objective To explore the application effect of umbilical foramen fascia platform approach in gynecological single-port laparoscopy.Methods From March 2021 to March 2023,60 patients with benign gynecological diseases admitted to our hospital were selected and divided into three groups according to the patient's wishes.Among them,20 patients underwent single-hole laparoscopic surgery on the umbilical Kangji single-hole Port approach platform(group A),20 patients underwent single-hole laparoscopic surgery on the umbilical fascia approach platform(group B),and 20 patients underwent multi-hole laparoscopic surgery(group C).The clinical related indexes,scar condition,wound healing degree and complication rate of the three groups were compared.Results There was no difference in operation time,postoperative anal exhaust time,hospitalization time,intraoperative blood loss,wound healing degree and complication rate among the three groups(P>0.05).There was no difference in the operation cost between group B and group C(P>0.05).The operation cost of group B and group C was lower than that of group A(P<0.05).One month after operation,the aesthetic satisfaction of incision in group B was higher than that in group A and group C(P<0.05).The aesthetic satisfaction of incision in group A was higher than that in group C(P<0.05).At 1 day after operation,there was no difference in pain scores among the three groups(P>0.05).At 3 days after operation,the pain score of group B was lower than that of group A and group C(P<0.05).At 7 days after operation,there was no difference in the Vancouver Scar Scale(VSS)scores among the three groups(P>0.05).One month after operation,the VSS score of group B was lower than that of group A and group C(P<0.05).The VSS score of group A was lower than that of group C(P<0.05).Conclusion The application of different approaches in gynecological single-hole laparoscopy can successfully complete LESS.Among them,the transumbilical fascial platform approach can reduce the impact of scar on the appearance withou
