摘要
目的:分析在疾病诊断相关分类(DRGs)支付改革下急性阑尾炎患者住院费用的影响因素,并构建DRGs病例组合方案。方法:选取2018年1月—2021年12月徐州医科大学附属医院收治的急性阑尾炎患者200例作为研究对象,按照住院费用分为高费用组(≥8783.27元)和低费用组(<8783.27元),各100例。分析其临床资料,总结在DRGs支付改革下急性阑尾炎患者住院费用的影响因素,构建DRGs病例组合。结果:影响患者住院费用的主要因素包括是否手术、住院时间、年龄、是否存在并发症、是否存在合并症,200例急性阑尾炎患者共形成8个DRGs组合及相应的住院费用标准。结论:急性阑尾炎患者DRGs住院费用标准能够为医院决策提供参考依据,影响患者住院费用的主要因素包括是否手术、住院时间、年龄等。
Objective:To analyze the influencing factors of hospitalization costs in patients with acute appendicitis under the reform of Diagnosis-Related Groups(DRGs)payment.Methods:A total of 200 patients with acute appendicitis admitted to Affiliated Hospital of Xuzhou Medical University from January 2018 to December 2021 were selected as the study subjects.The patients were divided into high cost group(≥8783.27 yuan)and low cost group(<8783.27 yuan)according to the hospitalization cost,with 100 cases in each group.The clinical data were analyzed,and the influencing factors of hospitalization cost in acute appendicitis patients under the reform of DRGs payment were summarized.DRGs case mix was constructed.Results:The main factors of hospitalization cost included the surgery,length of hospital stay,age,complications,comorbidities.There were a total of 8 DRGs mixes and corresponding hospitalization cost standards in 200 patients with acute appendicitis.Conclusion:The hospitalization cost standard of DRGs in patients with acute appendicitis can provide a reference basis for hospital decisionmaking,and the main factors affecting the hospitalization cost include the surgery,length of hospital stay,age and other factors.
