摘要
目的探究针刺联合活血通络方对气虚血瘀型卒中后偏瘫患者的临床治疗价值。方法选择营口市中医院2019年1月—2020年6月收治的86例气虚血瘀型卒中后偏瘫患者为研究对象,以随机数字表法将其分为对照组、观察组,各43例,分别实施中医针刺治疗、中医针刺+活血通络方治疗,比较2组的治疗效果、脑血流变化(颈动脉最小血流速度、脑血管外周阻力)、肢体运动功能(Fugl-Meyer)水平。结果观察组治疗总有效率为97.67%(42/43),高于对照组的81.40%(35/43)(P<0.05);治疗后,观察组颈动脉最小血流速度水平较对照组高,脑血管外周阻力水平较对照组低(P<0.05)且Fugl-Meyer中上肢、下肢功能评分较对照组高(P<0.05)。结论对气虚血瘀型卒中后偏瘫患者在常规治疗的基础上,应用针刺联合活血通络方,可提升治疗效果,改善患者脑血流状态及肢体运动功能。
Objective To explore the clinical value of acupuncture combined with Huoxue Tongluo formulas in the treatment of post-stroke hemiplegia of qi deficiency and blood stasis type.Methods 86 patients with post-stroke hemiplegia with qi deficiency and blood stasis in Yingkou Hospital of Traditional Chinese Medicine from January 2019 to June 2020 were selected as the research ob-jects.They were randomly divided into control group and observation group,with 43 cases in each group.They were treated with single acupuncture and acupuncture combined with Huoxue Tongluo formulas respectively.The therapeutic effect,changes of cerebral blood flow(minimum blood flow velocity of carotid artery,peripheral resistance of cerebrovascular)and the level of limb motor function(Fugl-meyer)were compared between the two groups.Results The total effective rate of observation group was 97.67%(42/43),which was higher than that of control group(81.40%,35/43)(P<0.05).After treatment,the minimum carotid blood flow velocity of the obser-vation group was higher than that of the control group,the peripheral cerebrovascular resistance level was lower than that of the control group(P<0.05),and the functional scores of the upper and lower limbs of the Fugl-Meyer group were higher than that of the control group(P<0.05).Conclusion On the basis of routine treatment of post-stroke hemiplegia of qi deficiency and blood stasis type,acu-puncture combined with Huoxue Tongluo formulas can improve the therapeutic effect,cerebral blood flow and limb motor functionon.
