摘要
目的了解基层医疗机构门诊处方的用药情况及处方质量,促进医务人员合理使用药物。方法随机抽取2021年1—12月陕西省渭南市刘集镇中心卫生院门诊处方,处方用药分析每月抽取100张,共1200张;处方点评分析每月抽取30张,共360张。根据《处方管理办法》、药品说明书、处方点评细则等对处方的合理性进行分析。结果处方疾病诊断以慢性病为主,主要为高血压和糖尿病;次均处方费用为39.69元,平均每张处方用药数为1.45种;药物的使用方式主要为口服;基本药物的使用按治疗类别划分,使用量最高的为心脑血管用药,占41.12%;销量最高的药品为治疗糖尿病和高血压的药品。基层医疗机构处方规范性较差,不合理处方占31.11%,其中不规范处方最多。结论基本药物制度在基层医疗机构得到较好的实施,基层医疗机构不合理处方占比高,需加强医务人员的合理用药培训,保障基层用药安全、合理。
Objective To understand the drug use situation and prescription quality of outpatient prescriptions in pri⁃mary medical institutions,and promote the rational use of drugs by medical staff.Methods Outpatient prescriptions were randomly selected from the Central Health Center of Liuji Town,Wenan City,Shaanxi Province from January to December 2021,and 100 prescriptions were selected every month,a total of 1200 prescriptions.A total of 360 pre⁃scriptions were selected from 30 prescriptions per month.The rationality of prescription was analyzed according to Pre⁃scription Management Measures,drug instructions,and prescription comment rules.Results The prescription diseases were mainly chronic diseases,mainly hypertension and diabetes.The average prescription cost was 39.69 yuan,and the average number of drugs per prescription was 1.45 kinds.The use of drugs is mainly oral.The use of essential drugs was divided by treatment categories,the highest use was cardio-cerebrovascular drugs,accounting for 41.12%.The top sellers were drugs for diabetes and high blood pressure.The prescription standard of primary medical institu⁃tions was poor,and irrational prescription accounted for 31.11%,among which the non-standard prescription was the most.Conclusion The system of essential drugs is well implemented in primary medical institutions,and irrational prescriptions account for a high proportion in primary medical institutions.It is necessary to strengthen the training of medical staff on rational drug use to ensure the safety and rationality of drug use at primary level.
