摘要
目的探究椎间孔镜微创术在治疗椎间盘突出症患者中的临床观察效果及对腰部活动度的影响观察。方法选取2022年1月—2022年12月郑州大学第一附属医院骨科收治的100例患者,随机分为对照组与试验组,对照组采取传统开放式椎板切除术治疗,试验组采取椎间孔镜微创术治疗,每组各50例。对比两组患者的围术期相关指标、治疗效果、术后并发症发生率、术前术后疼痛程度以及腰部活动度。结果试验组患者的手术时间、住院时间与手术切口均明显少于对照组,手术中出血量也少于对照组(P<0.05);试验组患者临床治疗效果(96.00%)优于对照组(64.00%)(P<0.05);试验组患者术后疼痛程度显著小于对照组(P<0.05);试验组术后腰椎前、后、左、右屈伸活动度均高于对照组(P<0.05);试验组术后并发症发生率(0.04%)明显低于对照组(34.00%),组间差异有统计学意义(P<0.05)。结论与传统开放式椎板切除术治疗腰椎间盘突出症患者相比,椎间孔镜微创术能够显著缩短手术时长、改善围术期相关指标与治疗效果、提高患者术后腰椎活动度、减轻患者疼痛程度,值得在临床中进一步推广应用。
Objective To explore the clinical observation effect of foraminoscopic microsurgery in the treatment of pa-tients with disc herniation and the effect of lumbar motion observation.Methods A total of 100 patients admitted to the Department of Orthopedics of the First Affiliated Hospital of Zhengzhou University from January 2022 to December 2022 were randomly divided into control group and experimental group.The control group was treated with traditional open laminectomy,and the experimental group was treated with foraminal microsurgery,with 50 cases in each group.The perioperative indexes,therapeutic effect,postoperative complication rate,preoperative and postoperative pain de-gree and lumbar motion were compared between the two groups.Results The operation time,hospital stay and inci-sion of experimental group were significantly less than that of control group,and the amount of blood loss during opera-tion was also less than that of control group(P<0.05).The clinical therapeutic effect of experimental group(96.00%)was better than that of control group(64.00%)(P<0.05).Postoperative pain degree of experimental group was significantly lower than that of control group(P<0.05).The anterior,posterior,left and right flexion and extension motion of lumbar spine in experimental groups were higher than those in control group(P<0.05).The in-cidence of postoperative complications in experimental group(0.04%)was significantly lower than that in control group(34.00%),and the difference between groups was statistically significant(P<0.05).Conclusion Com-pared with traditional open laminectomy in the treatment of patients with lumbar disc herniation,foraminal microsurgery can significantly shorten the operation duration,improve perioperative indexes and therapeutic effects,improve postoperative lumbar motion,and reduce the degree of pain of patients,which is worthy of further promotion and application in clinical practice.
