摘要
目的:分析桡骨远端骨折(DRF)掌侧钢板内固定后关节僵硬发生的影响因素,以便临床采取相关干预措施。方法:回顾性分析2019年8月—2021年8月于江西省中西医结合医院行掌侧钢板内固定治疗的100例DRF患者的临床资料,统计其术后关节僵硬发生情况,另收集所有患者的年龄、性别等一般资料,以多因素logistic回归分析模型分析DRF患者行掌侧钢板内固定治疗后关节僵硬发生的影响因素。结果:100例行掌侧钢板内固定治疗的DRF患者,术后发生关节僵硬26例,发生率为26.00%(26/100)。单因素分析显示:发生组和未发生组性别、体重指数(BMI)、骨折部位、受伤到手术时间比较,差异均无统计学意义(P>0.05);发生组年龄≥60岁、外固定时间>4周、功能锻炼不规律、术前肿胀程度为中重度占比均高于未发生组,差异均有统计学意义(P<0.05)。多因素logistic回归分析显示:年龄≥60岁、外固定时间>4周、功能锻炼不规律、术前中重度肿胀为DRF患者行掌侧钢板内固定治疗后发生关节僵硬的影响因素(P<0.05)。结论:DRF患者行掌侧钢板内固定治疗后关节僵硬发生率较高,而年龄≥60岁、外固定时间>4周、功能锻炼不规律、术前中重度肿胀为术后关节僵硬发生的影响因素,针对此临床需对上述因素予以高度重视,并施以针对性的干预措施,以加快患者恢复,减少术后关节僵硬的发生。
Objective:To analyze the influencing factors of joint stiffness after internal fixation with volar plate of distal radius fracture(DRF)so as to take relevant intervention measures in clinic.Method:The clinical data of 100 patients with DRF who underwent internal fixation with volar plate in Jiangxi Hospital of Integrated Traditional and Western Medicine from August 2019 to August 2021 were retrospectively analyzed,and the incidence of postoperative joint stiffness was counted.In addition,the age,gender and other general data of all patients were collected.Multivariate logistic regression analysis model was used to analyze the influencing factors of joint stiffness in DRF patients after internal fixation with volar plate.Result:Among 100 DRF patients treated with internal fixation with volar plate,26 cases developed joint stiffness after operation,the incidence rate was 26.00%(26/100).Univariate analysis showed that there were no significant differences in gender,body mass index(BMI),fracture site,injury to operation time between occurrence group and non-occurrence groups(P>0.05).The proportion of age≥60 years old,external fixation time>4 weeks,irregular functional exercise and the degree of preoperative edema was moderate to severe in the occurrence group were higher than those in the non-occurrence group,the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that age≥60 years old,external fixation time>4 weeks,irregular functional exercise and moderate to severe preoperative edema were the influencing factors for joint stiffness in DRF patients after internal fixation with volar plate(P<0.05).Conclusion:The incidence of joint stiffness in DRF patients after internal fixation with volar plate is high,and age≥60 years old,external fixation time>4 weeks,irregular functional exercise and moderate to severe preoperative edema are the influencing factors for the occurrence of postoperative joint stiffness.In view of this clinical need to attach great importance to
