摘要
目的分析重庆市第五届住院医师规范化培训临床技能竞赛的腹腔镜外科技能竞赛成绩,检验重庆市住培医师的腹腔镜外科技能培训效果,进一步反馈住培医师在腹腔镜外科技能培训中存在的问题,提高住培医师的腹腔镜外科技能培训教学方法。方法选取参加2021年12月重庆市第五届住院医师规范化培训临床技能竞赛初赛的34支参赛单位为研究对象,筛选出每个队伍参加腹腔镜外科技能考试站点的竞赛成绩。采用问卷调查和资料分析方法对技能操作考核结果进行横断面调查和统计学分析。结果34支参赛队伍中24支(70.6%)认为考核项目竞赛内容较难,6支(17.6%)认为此考核项目难度适中,4支(11.8%)认为此项目比较容易。28支队伍(82.3%)表示接触过腹腔镜模拟器的训练,但训练时间有限。6支参赛队伍(17.7%)表示从未培训过。全体考核成绩为0~79.25分,平均41.50分。按照评分标准分为5个维度评分项成绩,术前沟通项得分普遍较低,操作动作及操作质量维度表现上较为均衡,完成度项差异性较大。结论住培医师的腹腔镜外科操作技能急待提高,理论与外科技能操作融合及临床思维能力有待加强。各培训基地应重视腹腔镜外科技能培训项目的规范化开展。
Objective To analyze the results of laparoscopic surgery skills competition in the fifth clinical skills competition for standardized training of resident physicians in Chongqing,to test the effect of surgical skills training for residents in Chongqing,to further feed back the problems of residents in surgical skills training and improve the teaching methods of laparoscopic surgery skills training for residents.Methods 34 participating units in the preliminary competition of the fifth clinical skills competition for standardized training of residents in Chongqing in December 2021 were selected as the research objects,and the competition results of each team participating in the laparoscopic surgery skills examination site were selected.Questionnaire survey and data analysis were used to conduct cross-sectional survey and statistical analysis on the results of skill operation assessment.Results Among the 34 teams,24 teams(70.6%)thought that the competition content of the assessment item was difficult,6 teams(17.6%)thought that the difficulty of the assessment item was moderate,and 4 teams(11.8%)thought that the item was relatively easy.28 teams(82.3%)said they had been exposed to laparoscopic simulator training,but the training time was limited.Six teams(17.7%)said they had never been trained.The score of all assessment scores was 0-79.25,with an average was 41.50.According to the scoring criteria,the scores were divided into five dimensions.The scores of preoperative communication items were generally low,the performance of operation action and operation quality dimensions were relatively balanced,and the differences of completion items were large.Conclusion Laparoscopic surgery skills of residents need to be improved urgently,and the fusion of theory and surgical skills,clinical thinking ability need to be strengthened.All training bases should pay attention to the standardization of laparoscopic surgery skill training programs.
