摘要
目的 报道个性化运动处方联合膝三针促进飞行员前交叉韧带损伤多次术后快速康复1例。方法 该患者两次扭伤右膝关节,提示前交叉韧带损伤,分别行前交叉韧带重建术和紧缩术,术后未系统康复,遗留右膝关节肿胀、疼痛及活动受限,经个性化运动处方联合膝三针综合康复治疗2周。结果 治疗后症状消失,右膝关节功能显著改善。飞行员多次前交叉韧带损伤提示其肌骨系统存在薄弱部分,需要评定运动系统,制定个性化运动处方,通过改善弱链可快速改善肢体功能。而在运动前联合使用膝三针可缓解关节肿胀、疼痛及活动受限。结论 个性化运动处方联合膝三针对促进前交叉韧带损伤多次术后快速康复具有临床可行性。本例提示:结合中国的传统康复手段,未来形成中国特色加速康复外科(enhanced recovery after surgery,ERAS),但需要一系列的循证医学研究证实和临床运用。此外,针对一些少见病的术后康复,利用ERAS理念结合个性化原则,可探索适用、有效的综合手段进行精准康复治疗。
Objective To report a case of one pilot's rapid recovery after multiple operations of anterior cruciate ligament injury by individualized exercise prescription combined with Three-knee-acupoint acupuncture.Methods The patient sprained the right knee twice indicating the injury of the anterior cruciate ligament.The reconstruction and contraction of the anterior cruciate ligament were performed respectively.After the surgery,there was no systematic recovery,leaving the swelling,pain and activity limitation of the right knee joint.The patient was treated with individualized exercise prescription combined with Three-knee-acupoint acupuncture for two weeks.Results After treatment,the symptoms disappeared and the function of the right knee joint improved significantly.Multiple anterior cruciate ligament injuries in the pilot suggested that there were weak parts in his musculoskeletal system.It was necessary to assess the motor system,develop individualized exercise prescription,and quickly improve limb function by improving weak chains.The combined application of Three-knee-acupoint acupuncture before exercise could alleviate joint swelling,pain,and limited mobility.Conclusion It is clinically feasible that individualized exercise prescription combined with Three-knee-acupoint acupuncture can promote rapid rehabilitation after multiple operations of anterior cruciate ligament injury.This case suggests that the enhanced recovery after surgery(ERAS)with Chinese characteristics will be formed in the future by combining the traditional means of rehabilitation in China,but it needs a series of evidence-based medicine research confirmation and clinical application.In addition,for postoperative rehabilitation of some rare diseases,the ERAS concept combined with individualized principles can be used to explore applicable and effective comprehensive methods for precise rehabilitation treatment.
