3D-STI技术评估完全性左束支传导阻滞患者左心室同步性的价值

Evaluation of left ventricular synchronization by three dimensional speckle tracking imaging technique in patients with complete left bundle branch block

摘要 目的 :探讨三维斑点追踪成像(three dimensional speckle tracking imaging,3D-STI)技术评估完全性左束支传导阻滞患者左心室同步性的价值。方法:选取2019年8月至2020年8月某院收治的74例完全性左束支传导阻滞患者,根据左心室射血分数的不同分为A组(左心室射血分数≥50%,38例)和B组(左心室射血分数<50%,36例),并选取同期体检无异常的30例健康者作为对照组。采用3D-STI技术测量左心室同步性相关参数,采用方差检验分析各组相关参数的变化情况。结果:与对照组相比,A组患者左心室收缩末期内径增大,B组患者左心房前后径、左心室舒张末期内径、左心室收缩末期内径、舒张末期室间隔厚度、左心室后壁厚度增大,左心室射血分数降低,差异均有统计学意义(P<0.05)。与A组相比,B组患者左心室舒张末期内径增大、左心室射血分数降低,差异均有统计学意义(P<0.05)。与对照组相比,A、B 2组患者的收缩期左心室整体纵向峰值应变、整体环向峰值应变、整体径向峰值应变、整体面积峰值应变降低,且B组患者较A组患者各应变参数更低,差异均有统计学意义(P<0.05)。各组患者左心室局部收缩期基底段、中间段、心尖段应变参数逐渐降低。与对照组相比,A组患者和B组患者左心室同步性三维参数显著升高,且B组较A组更高,差异均有统计学意义(P<0.05)。结论:3D-STI可准确获取完全性左束支传导阻滞患者的左心室心肌运动参数,对左心室同步性具有较好的评估价值。 Objective To investigate the value of three dimensional speckle tracking imaging(3D-STI)technique for evaluating left ventricular synchronization in the patients with complete left bundle branch block.Methods Totally 74 patients with complete left bundle branch block admitted to some hospital from August 2019 to August 2020 were selected and divided into Group A(left ventricular ejection fraction≥50%,38 cases)and Group B(left ventricular ejection fraction<50%,36 cases)based on the difference of left ventricular ejection fraction,and 30 healthy people undergoing physical examination during the same period were enrolled into a control group.The parameters related to left ventricular synchronization were measured with 3D-STI technique,and the changes of the related parameters in each group were analyzed with variance test.Results Compared with the control group,group A had the left ventricular end-systolic inner diameter rose,and group B had the left atrial anteroposterior diameter,left ventricular end-diastolic inner diameter,left ventricular end-systolic inner diameter,end-diastolic interventricular septum thickness and left ventricular posterior wall thickness increased while the left ventricular ejection fraction decreased(P<0.05).Group B had the left ventricular end-diastolic diameter raised while the left ventricular ejection fraction lowered when compared with group A(P<0.05).Group A and B had the overall longitudinal peak strain of the left ventricle during systole,the overall peak circumferential strain of the left ventricle during systole,the overall peak radial strain of the left ventricle during systole and the overall peak strain of the left ventricle during systole decreased when compared with the control group,and the strain parameters in group B were lower than those in group A,with all the differences being statistically significant(P<0.05).The strain parameters of the basal segment,middle segment and apical segment of left ventricular local systole in each group got lowered gradually(P<0.05).Grou
