摘要
目的研究酚类暴露与估计肾小球滤过率(estimated glomerular filtration rate,eGFR)的关联,探讨氧化应激是否在酚类暴露和eGFR关系中存在中介效应。方法在华中科技大学同济医学院附属同济医院健康体检人群中招募414名参与者,收集其健康信息,采集其空腹晨尿并检测尿液中的3种酚类物质和3种氧化应激标志物的浓度。eGFR由慢性肾脏病流行病学协作组肾小球滤过率估算方程计算得到。采用多元线性回归模型评估酚类暴露、氧化应激标志物和eGFR的关联,通过中介效应分析评估氧化应激标志物在酚类暴露与eGFR关联中的中介作用。结果研究对象的年龄中位数为59岁,其中32.85%为女性。尿液双酚A(bisphenol A,BPA)、双酚F(bisphenol F,BPF)、三氯生(triclosan,TCS)、8-羟基脱氧鸟苷(8-hydroxydeoxyguanosine,8-OHdG)、4-羟基-2-壬烯-巯基乙酸(4-hydroxy-2-nonene-mercaptoacetic acid,HNE-MA)和8-异前列腺素F_(2α)(8-isoprostaglandin F_(2α),8-isoPGF_(2α))的浓度中位数分别为0.92、0.12、0.64、5.87、14.25和5.42 ng/mL。多元线性回归分析显示,尿BPA浓度与eGFR呈负向关联[β(95%CI)=-0.018(-0.030,-0.005),P=0.006],尿BPF、TCS与eGFR的关联无统计学意义。尿BPA、BPF、TCS与氧化应激标志物HNE-MA均呈显著正向关联(均P<0.05)。HNE-MA在BPA与eGFR之间的关联中存在显著的中介效应[β(95%CI)=-0.003(-0.006,-0.001),P=0.045],其中介效应比例为16.67%。结论BPA暴露与肾功能指标eGFR水平下降具有相关性。HNE-MA在BPA暴露与eGFR水平间的关联中具有中介效应,提示BPA可能通过脂质过氧化损伤导致肾功能下降。
Objective To investigate the relationship between phenol exposure and estimated glomerular filtration rate(eGFR),and how it is influenced by the mediating effect of oxidative stress biomarkers.Methods A total of 414participants were recruited from the physical examination population,and their health information and fasting morning urine samplesFontType:T were collected.The concentrations of three phenolic substances and three oxidative stress markers in the urine were detected.The outcome measure eGFR was estimated by the chronic kidney disease epidemiology collaboration(CKD-EPI)equation.Multiple linear regression model was used to evaluate the association between phenol exposure,oxidative stress and eGFR.Mediation analysis was applied to estimate the mediating role of oxidative stress markers in the relationship between phenol exposure and eGFR.Results A total of 414subjects with a median age of 59(32.85%females)were included.The median concentrations of urinary bisphenol A(BPA),bisphenol F(BPF),and triclosan(TCS),8-hydroxy-deoxyguanosine(8-OHdG),4-alkyl-2-nonenalde-hydrophouric acid(HNE-MA),and 8-isoprostaglandin F_(2α)(8-iso-PGF_(2α))were 0.92,0.12,0.64,5.87,14.25,and 5.42ng/mL,respectively.Multiple linear regression analysis showed that urinary BPA concentration was negatively correlated with eGFR[β(95%CI)=-0.018(-0.030,-0.005),P=0.006]and the association of urinary BPF and TCS with eGFR was not statistically significant.Urinary BPA,BPF,TCS and oxidative stress marker HNE-MA showed significant positive association(P<0.05).HNE-MA had a significant mediating effect in the association between BPA and eGFR[β(95%CI)=-0.003(-0.006,-0.001),P=0.045],with a mediating ratio of 16.67%.Conclusion BPA exposure was correlated with decreased levels of renal function index eGFR.HNE-MA had a mediating effect in the association between BPA exposure and eGFR levels,suggesting that BPA may cause renal function decline through lipid peroxidation damage.
