慢性阻塞性肺疾病急性加重期患者撤机拔管失败的风险因素及护理对策研究进展

摘要 机械通气是慢性阻塞性肺疾病急性加重期(AECOPD)患者常用且有效的治疗方法,但长期使用呼吸机辅助通气会出现一系列并发症,当患者的病情出现好转,应尽早考虑呼吸机脱机及拔除气管插管,可减少机械通气相关并发症、提高患者预后。但是如何识别拔管失败高风险的患者、掌握最佳的拔管时机、保证成功拔除气管插管又是临床医护人员面临的一大难题。因此,对AECOPD患者机械通气撤机拔管失败的风险因素及护理对策的研究现状进行综述,以期为医护人员全面评估患者病情、识别拔管失败高风险人群、实施针对性治疗和目标导向性的护理干预提供依据,也为进一步建立撤机拔管失败风险预测模型提供参考和借鉴。 Mechanical ventilation is chronic obstructive pulmonary diseases acute exacerbation(AECOPD)patients commonly used and effective treatment method,but the long-term use of breathing machine auxiliary exchange meetings appear a series of complications,when the patient once appear beter,should be considered as early as possible out ventilator weaning and tracheal intubation,mechanical ventilation is to reduce the related complications,measures to improve the prognosis of patients.However,how to identify patients with high risk of extubation failure,grasp the best extubation time,and ensure successful extubation is also a major problem facing clinical medical staff.Therefore,this paper reviews the research status of risk factors and nursing countermeasures of mechanical ventilation extubation failure in patients with AECOPD,in order to provide medical staff with a comprehensive assessment of patients'condition,identify high-risk groups of extubation failure,and provide a basis for the implementation of targeted treatment and goal-oriented nursing intervention.It also provides reference for further establishment of risk prediction model of weaning and extubation failure.
