摘要
目的分析重复经颅磁刺激(rTMS)联合帕罗西汀对帕金森合并抑郁(PD-D)患者认知、情绪及血清5-羟色胺(5-HT)、白细胞介素-1β(IL-1β)水平的影响。方法选取2019年2月~2022年3月我院诊治的PD-D患者102例,按随机数字表法分为对照组与研究组各51例,对照组采用帕罗西汀治疗,研究组采用rTMS联合帕罗西汀治疗,比较两组患者的临床疗效、汉密尔顿抑郁量表(HAMD)评分、抑郁自评量表(SDS)评分、蒙特利尔认知评估量表(MoCA)评分、简易智能精神状态检查量表(MMSE)评分、血清5-HT与IL-1β水平及不良反应状况。结果研究组治疗有效率高于对照组(P<0.05);研究组HAMD、SDS评分及血清IL-1β水平均低于对照组(P<0.05);研究组MMSE、MoCA评分及血清5-HT水平均高于对照组(P<0.05);研究组不良反应发生率低于对照组(P<0.05)。结论rTMS联合帕罗西汀治疗PD-D,可提高疗效,有效改善患者抑郁症状与认知状态,调节血清5-HT与IL-1β水平并减少不良反应发生情况。
Objective To analyze effects of repeated transcranial magnetic stimulation(rTMs)combined with paroxetine on cognitive status,depression symptoms,serum 5-hydroxytryptamine(5-HT)and interleukin-1β(IL-1β)in patients with Parkinson's disease and depression(PD-D).Methods 102 patients with PD-D treated in the hospital were enrolled and random number table method divided into control group(paroxetine)and study group(rTMS+paroxetine)between February 2019 and March 2022,51 cases in each group.Clinical curative effect,scores of Hamilton Depression Scale(HAMD),Self-rating Depression Scale(SDS),Montreal Cognitive Assessment Scale(MoCA)and Mini-Mental State Examination(MMSE),levels of serum 5-HT and IL-1β,and adverse reactions were compared between the two groups.Results The response rate of treatment was higher in study group than control group(P<0.05).HAMD and SDS scores,and IL-1βwere lower in study group than control group(P<0.05),while MoCA and MMSE scores,and 5-HT were higher in study group(P<0.05).The incidence of adverse reactions was lower in study group than control group(P<0.05).Conclusion The rTMS combined with paroxetine can improve curative effect,effectively improve depression symptoms and cognitive status,regulate levels of serum 5-HT and IL-1β,and reduce adverse reactions in PD-D patients.
