摘要
目的探讨比较胆囊切除术采用七氟烷、丙泊酚联合芬太尼麻醉的效果与安全性。方法选择2022年1-10月在本院接受腹腔镜胆囊切除术治疗的患者60例作为研究对象,根据随机信封抽签法分作对照组(30例)与观察组(30例),对照组麻醉方案为丙泊酚复合芬太尼;观察组麻醉方案为七氟烷复合芬太尼,记录各组患者临床麻醉及苏醒用时情况,监测各组患者麻醉前后不同时间节点心率、血压及血氧饱和度指标变化,评估各组患者术后不同时间节点认知功能恢复情况,观察统计两组患者出现不良反应的表现与总发生率。 结果观察组患者麻醉诱导时间及术后苏醒、拔管时间均短于对照组(P<0.05),插管后5min、气腹5min、拔管后5min,观察组患者心率、平均动脉压均低于对照组(P<0.05),血氧饱和度水平与对照组相比差异无统计学意义(P>0.05)。术后观察组患者认知功能评分均高于对照组(P<0.05)。两组患者不良反应总发生率比较差异无统计学意义(P>0.05)。结论临床开展腹腔镜胆囊切除术期间采用七氟烷复合芬太尼进行麻醉具有较好麻醉效果,同时减轻对患者的影响,有助于术后患者认知功能得到更快恢复,安全性佳,值得推广。
Objective To investigate effect and safety of sevoflurane,propofol and fentanyl anesthesia for cholecystectomy.Methods The paper chose 60 patients with laparoscopic cholecystectomy in our hospital from January to October 2022 as research objects,and divided them into control group(30 cases)and observation group(30 cases)according to random envelope drawing method.Anesthesia scheme of control group was propofol combined with fentanyl;that of observation group was sevoflurane,propofol and fentanyl.Clinical anesthesia and recovery time in each group was recorded,changes of heart rate,blood pressure and oxygen saturation at different time points before and after anesthesia were monitored in each group.Recovery of cognitive function at different time points after surgery was evaluated in each group.Performance and total incidence of adverse reactions in two groups was observed and counted.Results Anesthesia induction time,postoperative recovery time and extubation time in observation group was shorter than control group(P<0.05).Cognitive function scores of postoperative observation group were higher than control group(P<0.05).There was no statistically significant difference in total incidence of adverse reactions between two groups(P>0.05).Conclusion Sevoflurane,propofol and fentanyl anesthesia can achieve good anesthetic effect,reduce effect on patients,help faster recovery of postoperative cognitive function of patients with good safety during clinical laparoscopic cholecystectomy,which is worthy of promotion.
