1.5T MRI对单侧不可复性关节盘前移位定量分析

摘要 目的采用1.5T MRI对单侧不可复性关节盘前移位(ADDWoR)患者双侧颞颌关节盘前后带厚度及关节盘长度变化进行分析,探讨其临床价值。方法回顾性分析46例单侧ADDWoR患者MRI图像,行双侧颞下颌关节MRI成像,斜矢状位T2WI分别测量双侧关节盘前、后带厚度及关节盘长度,结合斜矢状及冠状位T2WI、PDWI,统计患侧是否合并髁突骨质改变及有无关节腔积液,对两侧颞下颌关节数值进行分析。结果46例患者健侧关节盘长度(10.74±1.56)mm值>患侧关节盘长度(9.34±1.99)mm(P<0.05);健侧关节盘前带厚度(2.58±0.60)mm<患侧关节盘前带厚度(2.81±0.75)mm但差异无统计学意义(P>0.05);健侧关节盘后带厚度(2.66±0.79)mm<患侧关节盘后带(3.58±0.73)mm(P<0.05);ADDWoR伴发髁突骨质改变者35例(76.1%),伴发关节腔积液者27例(58.7%),同时伴有髁突骨质改变及关节腔积液者18例(39%)。结论MRI能客观显示颞下颌关节盘形态变化,通过对ADDWoR患者关节盘定量研究,对临床诊断颞下颌关节紊乱具有十分重要的临床价值。 Objective To analyze the changes in the thickness of the anterior and posterior band of bilateral temporomandibular joint(TMJ)discs and the length of TMJ discs in patients with unilateral non-reducible anterior disc displacement without reduction(ADDWoR)using 1.5T magnetic resonance imaging(MRI),and to explore its clinical value.Methods MRI images of 46 patients with unilateral non-reducible ADDWoR,including 14 males and 32 females,were retrospectively analyzed.The patients were aged from 11 to 65 years old.The thickness of the anterior and posterior band of bilateral TMJ discs and the length of TMJ discs were measured on oblique sagittal and coronal T2 weighted image(T2WI)and proton-density weighted image(PDWI).Condylar bone changes and joint cavity effusion on the affected side were recorded.Imaging data of bilateral TMJ were statistically analyzed.Results In 46 patients with unilateral non-reducible ADDWoR of TMJ,the mean length of TMJ discs at the healthy side was significantly longer than that of the affected side([10.74±1.56]mm vs[9.34±1.99]mm,P<0.05).The mean thickness of the anterior band of bilateral TMJ discs at the healthy side was lower than that of the affected side,although no significant difference was detected([2.58±0.60]mm vs[2.81±0.75]mm,P>0.05),while that of the posterior band of bilateral TMJ discs at the healthy side was significantly lower than that of the affected side([2.66±0.79]mm vs[3.58±0.73]mm,P<0.05).ADDWoR combined with condylar bone changes,joint cavity effusion and both were found in 35(76.1%)cases,27(58.7%)cases,and 18(39%)cases,respectively.Conclusion MRI can objectively visualize TMJ diseases.A quantitative study of TMJ discs in patients with t ADDWoR has very important clinical value for the clinical diagnosis of TMJ disorders.
