术前CCL26血清水平对手术治疗慢性鼻-鼻窦炎的预后价值

摘要 目的探讨术前CCL26血清水平对手术治疗慢性鼻—鼻窦炎的预后价值。方法选取GEO数据库CRS患者转录组数据,通过差异表达基因筛选、免疫细胞浸润评分、基因富集分析等算法筛选到CRS相关嗜酸性粒细胞趋化因子。回顾我科手术治疗的CRS患者随访资料,分析术前趋化因子血清水平对合并鼻息肉是否具有诊断效能,进一步评估其对手术改善患者生活质量的预后潜力。结果计算CRSsNP组与CRSwNP组差异表达基因共125个(其中上调49个,下调76个)。CRSwNP组嗜酸性粒细胞及树突状细胞免疫评分明显高于CRSsNP组(P<0.05),嗜酸性粒细胞的迁移和趋化相关基因在CRSwNP组表达均上调(P<0.01)。通过交叉鉴定出2个差异表达的嗜酸性粒细胞趋化因子(CCL26、CCL13)。随访慢性鼻—鼻窦炎手术患者115例,术前血清CCL26水平对合并鼻息肉具有良好诊断价值(ROC=0.895),其表达水平与手术前后SNOT22评分差值呈负相关(β=-0.562,P<0.001)。结论嗜酸性粒细胞趋化因子CCL26参与慢性鼻—鼻窦炎发病机制,术前检测其血清水平对评估手术治疗可能具有一定预后价值。 Objective:To investigate the prognostic value of preoperative serum level of CCL26 in the treatment of chronic rhinosinusitis.Methods:The transcriptome data of CRS patients were selected from GEO database,and CRS-related eosinophil chemokines were screened by dfferential expression gene screening,immune cell infiltration score,gene enrichment analysis and other algorithms.The follow-up data of CRS patients treated by surgery in our department were reviewed to analyze whether preoperative chemokine serum levels had diagnostic efficacy for complicated nasal polyps,and to further evaluate their prognostic potential for improving the quality of life of patients undergoing surgery.Results:125 differentially expressed genes were calculated between CRSsNP group and CRSwNP group(49 up-regulated and 76 down-regulated).The immune scores of eosinophils and dendritic cells in CRSwNP group were significantly higher than those in CRSsNP group(P<0.05),and the expression of eosinophils migration and chemotactic related genes were up-regulated in CRSwNP group(P<0.01).Two differentially expressed eosinophilic chemokines(CCL26 and CCL13)were identified by cross-identification.During the follow-up of 115 patients with chronic rhinosinusitis,the preoperative serum CCL26 level had a good diagnostic value for complicated nasal polyps(ROC=0.895),and its expression level was negatively correlated with the difference of SNOT22 score before and after surgery(β=-0.562,P<0.001).Conclusion:Eosinophilic chemotactic factor CCL26 is involved in the pathogenesis of chronic rhinosinusitis.Preoperative detection of its serum level may have prognostic value in evaluating surgical treatment.
