摘要
目的分析儿童紫癜性肾炎(HSPN)临床特征及其相关危险因素。方法选取2020年1月至2022年10月中国人民解放军联勤保障部队第九二四医院收治的50例HSPN患儿作为观察组,将同期收治的50例过敏性紫癜患儿作为对照组。比较两组临床资料,分析HSPN临床特征,采用单因素及logistic多因素回归分析HSPN的相关危险因素。结果观察组年龄为7~14岁,临床症状为反复皮疹、关节肿痛、腹痛、血尿、蛋白尿等,发病集中在春季和冬季。两组年龄、皮疹反复、幽门螺杆菌感染、链球菌感染、血清胱抑素C(CysC)及尿微量白蛋白比较,差异均有统计学意义(P<0.05)。logistic多因素回归分析显示,年龄≥7岁、皮疹反复、幽门螺杆菌感染、链球菌感染、血清CysC高、尿微量白蛋白高是儿童HSPN发生的相关危险因素(P<0.05)。结论儿童HSPN的相关危险因素较多,临床需根据相关危险因素及早采取干预措施,以减少儿童HSPN的发生或降低肾损害程度,对于儿童HSPN的防治具有积极作用。
Objective To analyze the clinical characteristics and related risk factors of henoch schonlein purpura nephritis(HSPN)in children.Methods A total of 50 children with HSPN admitted to the 924^(th) Hospital of Chinese People’s Liberation Army Joint Logistic Support Force from January 2020 to October 2022 were selected as the observation group,and 50 children with allergic purpura admitted during the same period were selected as the control group.The clinical data of the two groups were compared,the clinical characteristics of HSPN were analyzed,and the related risk factors of HSPN were analyzed by univariate and logistic multivariate regression.Results Children in the observation group ranged in age from 7 to 14 years old.The clinical symptoms of the observation group were repeated rash,joint swelling pain,abdominal pain,hematuria,proteinuria,etc.The incidence was concentrated in spring and winter.There were significant differences in age,rash recurrence,Helicobacter pylori infection,streptococcus infection,serum cystatin C(CysC)and urinary microalbumin between two groups(P<0.05).The logistic multivariate regression analysis showed that age≥7 years,repeated rash,Helicobacter pylori infection,streptoc occus infection,high serum CysC and high urinary microalbumin were the related risk factors for HSPN in children(P<0.05).Conclusion There are many risk factors related to HSPN in children,and early intervention measures should be taken according to the related risk factors to reduce the occurrence of HSPN in children or reduce the degree of kidney damage,which has a positive effect on the prevention and treatment of HSPN in ch ildren.
