摘要
目的研究人类免疫缺陷病毒感染/获得性免疫缺陷综合征(HIV/ADIS)患者术后感染的相关危险因素,为预防HIV/AIDS患者机会性感染提供数据支持。方法回顾性分析2015年1月至2021年12月该院收治的41例发生术后切口感染(感染组)和130例未发生感染(非感染组)的HIV/AIDS患者的临床资料,比较2组患者一般情况及治疗相关指标,采用单因素和多因素logistic回归模型分析,筛选手术感染的独立危险因素。结果2组患者术前CD4^(+)T淋巴细胞计数、住院时间、切口类型比较,差异均有统计学意义(P<0.05);术前CD4^(+)T淋巴计数值小于200个/μL、Ⅲ类切口均是HIV/ADIS患者术后感染的独立危险因素(优势比=3.029、17.255,95%可信区间:1.350~6.798、4.549~65.443,P<0.05)。结论HIV/ADIS患者低免疫状态和Ⅲ类切口和术后感染密切相关,且为独立危险因素。建议对低免疫状态和Ⅲ类切口的HIV/AIDS患者进行规范和足量应用抗感染药物。
Objective To study the related risk factors of postoperative infection in patients with Human Immunodeficiency Virus/Acquired Immune Deficiency Syndrome(HIV/AIDS),and to provide data support for preventing opportunistic infections in HIV/AIDS patients.Methods The clinical data of 41 patients with HIV/AIDS who developed postoperative incisional infections(infected group)and 130 patients who did not develop infections(non-infected group)admitted to the hospital from January 2015 to December 2021 were retrospectively analyzed.The general conditions and treatment-related indicators of the two groups of patients were compared and univariate and multivariate logistic regression analysis were used to screen the independent risk factors for surgical infections.Results There were significant differences(P<0.05)in the preoperative CD4^(+)T lymphocyte count,stay length of hospital,and type of incision between the two groups of patients.Preoperative CD4^(+)T lymphocyte count less than 200 cells/μL and ClassⅢincision were independent risk factors for postoperative infection in HIV/AIDS patients(OR:3.029,17.255;95%CI:1.350-6.798,4.549-65.443;P<0.05).Conclusion The low immune status of HIV/AIDS patients and ClassⅢincisions were closely related to postoperative infections and were independent risk factors.It is recommended that HIV/AIDS patients with low immune status and ClassⅢincisions receive standardized and sufficient use of antimicrobial drugs.
