摘要
目的探讨脓毒症患者在重症监护室(ICU)内发生下肢深静脉血栓(LDVT)的危险因素。方法收集2020年10月至2022年9月于新疆医科大学第一附属医院就诊的147例脓毒症患者的临床资料,按照LDVT发生情况将其分为LDVT组(n=35)与NLDVT组(n=112)。比较两组患者的一般资料,分析脓毒症患者ICU内发生LDVT的危险因素。结果两组患者卧床时间、全身感染相关器官功能衰竭(SOFA)评分、格拉斯哥昏迷指数(GCS)评分、D-二聚体(D-D)水平比较,差异均有统计学意义(P<0.05)。多因素分析结果显示,GCS评分≥10.32分为脓毒症患者ICU内发生LDVT的保护因素(OR<1,P<0.05),卧床时间≥9.96 d、D-D≥521.85μg/L为脓毒症患者ICU内发生LDVT的危险因素(OR≥1,P<0.05)。结论卧床时间较长、D-D水平较高、GCS评分较低均是脓毒症患者ICU内发生LDVT的危险因素,临床上应重点观察,并积极给予预防性治疗,以降低患者LDVT发生风险。
Objective To study the risk factors of lower extremity deep vein thrombosis(LDVT)in patients with sepsis in intensive care unit(ICU).Method The clinical data of 147 patients with sepsis in the First Affiliated Hospital of Xinjiang Medical University from October 2020 to September 2022 were collected,they were divided into LDVT group(n=35)and NLDVT group(n=112)according to the occurrence of LDVT.The general data of the two groups were compared and the risk factors of LDVT of septic patients in ICU were analyzed.Result There were significant differences for bedtime,scores of sequential organ failure assessment(SOFA),Glasgow coma scale(GCS)and D-dimer(D-D)in two groups(P<0.05).Multivariate analysis showed that GCS score≥10.32 was the protective factor of LDVT in septic patients in ICU(OR<1,P<0.05),bedtime≥9.96 d and D-D≥521.85μg/L were the risk factors for the LDVT in patients with sepsis in ICU(OR≥1,P<0.05).Conclusion Longer bedtime,increased D-D and lower GCS scores are risk factors for LDVT in septic patients in ICU,which should be monitored closely,and preventive treatment should be given to reduce the risk of LDVT.
