中药灌肠联合无创正压通气治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭临床研究 被引量:2

Clinical Study on Enema with Chinese Medicine Combined with Non-Invasive Positive Pressure Ventilation for Chronic Obstructive Pulmonary Disease Complicated with TypeⅡRespiratory Failure

摘要 目的:观察中药灌肠联合无创正压通气治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的临床疗效。方法:选择100例COPD合并Ⅱ型呼吸衰竭患者为研究对象,按随机数字表法分为对照组与研究组各50例。对照组给予常规西医及无创正压通气治疗,研究组在对照组基础上给予中药灌肠治疗,2组均连续治疗7 d。比较2组治疗前后血清Nod样受体蛋白3(NLRP3)、热休克蛋白27(HSP27)、白细胞介素-27(IL-27)、高迁移率族蛋白B1(HMGB1)水平,呼气峰流速(PEF)、第1秒用力呼气容积(FEV1)、用力肺活量(FVC)水平,动脉血氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、血氧饱和度(SaO_(2))水平。结果:治疗前,2组HSP27、IL-27、NLRP3、HMGB1水平比较,差异无统计学意义(P>0.05);治疗后,2组HSP27、IL-27、NLRP3、HMGB11水平均降低(P<0.05),且研究组低于对照组(P<0.05)。治疗前,2组PaO_(2)、PaCO_(2)、SaO_(2)水平比较,差异无统计学意义(P>0.05);治疗后,2组PaO_(2)、SaO_(2)水平升高(P<0.05),且研究组高于对照组(P<0.05),2组PaCO_(2)水平降低(P<0.05),且研究组低于对照组(P<0.05)。治疗前,2组PEF、FEV1、FVC水平比较,差异无统计学意义(P>0.05);治疗后,2组PEF、FEV1、FVC水平升高(P<0.05),且研究组高于对照组(P<0.05)。结论:中药灌肠联合无创正压通气治疗COPD合并Ⅱ型呼吸衰竭患者,可抑制炎症,改善呼吸功能及血气指标。 Objective:To observe the clinical effect of the therapy of enema with Chinese medicine combined with non-invasive positive pressure ventilation on chronic obstructive pulmonary disease(COPD)complicated with typeⅡrespiratory failure.Methods:A total of 100 COPD patients complicated with typeⅡrespiratory failure were selected as the study objects,and were divided into the control group and the study group according to random number table method,with 50 cases in each group.The control group was given conventional western medicine and non-invasive positive pressure ventilation,and the study group was additionally given enema with Chinese medicine based on the treatment of the control group.Both groups were treated for seven days.The levels of serum Nod-like receptor protein 3(NLRP3),heat shock protein 27(HSP27),interleukin-27(IL-27),high mobility group protein B1(HMGB1),peak expiratory flow(PEF),forced expiratory volume in the first second(FEV1)and forced vital capacity(FVC),arterial partial pressure of oxygen(PaO2),partial pressure of carbon dioxide(PaCO2),and oxygen saturation(SaO2)were compared before and after treatment.Results:Before treatment,there was no significant difference in the comparison of the levels of HSP27,IL-27,NLRP3 and HMGB1 between the groups(P>0.05).After treatment,the levels of HSP27,IL-27,NLRP3 and HMGB11 in the two groups were decreased(P<0.05),and the levels in the study group was lower than those in the control group(P<0.05).Before treatment,there was no significant difference in the levels of PaO2,PaCO2 and SaO2 between the two groups(P>0.05).After treatment,the levels of PaO2 and SaO2 in the two groups were increased(P<0.05),and those in the study group were higher than those in the control group(P<0.05);the PaCO2 levels in the two groups were decreased(P<0.05),and the PaCO2 level in the study group was lower than that in the control group(P<0.05).Before treatment,there was no significant difference in the levels of PEF,FEV1 and FVC between the two groups(P>0.05).After treatment,the l
