摘要
目的 分析社会支持在广东农村老年糖尿病患者抑郁和用药依从性之间的中介作用。方法 采用抑郁量表、社会支持评定量表和用药依从性量表对1 495名老年人进行问卷调查。结果 农村老年糖尿病患者社会支持得分为(26.69±3.72),用药依从性得分为(4.98±0.99),用药依从性差的比例高达68.2%(1 019/1 495),抑郁的患病率为43.55%(651/1 495);用药依从性与抑郁呈负相关关系(r=-0.704,P<0.01),社会支持与用药依从性具有正相关关系(r=0.604,P<0.01),社会支持在抑郁与用药依从性之间具有部分中介效应,中介效应占总效应比例为17.62%。对年龄分层后,发现社会支持在抑郁和用药依从性之间的中介效应与年龄具有正相关关系。结论 抑郁可以直接对用药依从性产生负向作用,也可以通过社会支持产生间接产生中介作用。农村老年糖尿病群体,尤其高龄老年人,应被关注是否伴发抑郁等负向情绪,应充分利用社会支持以提高用药依从性。
Objective To analyze the mediating effect of social support on depression and medication adher-ence among elderly diabetic patients in rural Guangdong.Methods A questionnaire survey was conducted among 1495 elderly people with depression scale,social support rating scale and medication adherence scale.Results The social support score of rural elderly diabetic patients was(26.69±3.72),the medication compliance score was(4.98±0.99),the proportion of poor medication compliance was 68.2%(1019/1495),and the incidence of depression was 43.5%(651/1495);medication adherence was negatively correlated with depression(r=-0.704,P<0.01),while social support was positively correlated with medication adherence(r=0.604,P<0.01).There was a partial mediating effect of social support on depression and medication adherence,and the mediating effect accounted for 17.62%of the total effect.After stratifying by age,the mediating effect of social support on depression and medication adherence was found to be positively related to age.Conclusions Depres-sion can directly have a negative effect on medication adherence,and it can also have an indirect mediating effect through social support.Rural elderly people with diabetes,especially those with advanced age,should be watched on whether they are accompanied by negative emotions such as depression,and it is necessary to make full use of social support to improve medication compliance.
