Analysis of the Effect of CT-enhanced Scanning Localization Technique on the Dose of Radiotherapy for Middle and Lower Esophageal Cancer

摘要 目的分析CT增强扫描定位技术对中下段食管癌放疗剂量的影响。方法选取2019年8月—2022年8月盐城市第二人民医院接收的128例中下段食管癌患者为研究对象,所有患者均接受放疗,在放疗期间,予以患者进行CT平扫和CT增强扫描定位,比较CT平扫和CT增强扫描定位对应的肿瘤靶区剂量、计划靶区剂量、危及器官(心脏平均、肺部平均、脊髓平均、脊髓最大)剂量。结果CT增强扫描定位的肿瘤靶区剂量(68.12±0.99)Gy、计划靶区剂量(66.94±0.93)Gy低于CT平扫的(70.15±1.01)、(68.54±0.95)Gy,差异有统计意义(t=15.259,14.543,P<0.05);CT增强扫描定位的心脏平均剂量、脊髓平均剂量、脊髓最大剂量略低于CT平扫,肺部平均剂量略高于CT平扫,但差异无统计学意义(P>0.05)。结论CT增强扫描定位技术会引起中下段食管癌放疗剂量下降,但对器官正常组织的剂量影响少,在放疗过程中需考虑CT增强扫描定位的影响。 Objective To analyze the effect of CT-enhanced scanning positioning technique on the radiotherapy dose of middle and lower segment esophageal cancer.Methods 128 patients with middle and lower segment esophageal cancer admitted to the Yancheng Second People's Hospital from August 2019 to August 2022 were selected as the research object,and all patients received radiotherapy.During radiotherapy,patients were given CT plain and CT-enhanced scans for localization.The doses to the tumor target area,the planned target area dose,and the doses to the organs at risk(average dose to the heart,average dose to the lung,average dose to the spinal cord,and maximum dose to the spinal cord)were compared between CT scan and CT enhanced scan positioning.Results The tumor target area dose(68.12±0.99)Gy and planned target area dose(66.94±0.93)Gy for CT-enhanced scan localization were lower than those for CT plain scan(70.15±1.01)Gy and(68.54±0.95)Gy,the difference was statistically significant(t=15.259,14.543,P<0.05);the mean dose of heart,mean dose of spinal cord,and maximum dose of spinal cord for CT-enhanced scan localization were slightly lower than CT plain,and the mean dose of lung was slightly higher than CT plain,but the difference was not statistically significant(P>0.05).Conclusion The CT-enhanced scan localization technique causes a decrease in the dose of radiotherapy for middle and lower esophageal cancer,but has less effect on the dose of normal tissues of the organ,and the effect of CT-enhanced scan localization needs to be considered in the radiotherapy process.
