摘要
目的探讨利用二维与彩色多普勒超声(彩超)检查对早期诊断甲状腺乳头状癌的价值。方法选取2020年1月—2022年10月睢宁县人民医院超声医学科收治的甲状腺病变患者60例,均行二维超声与彩超检查,以病理诊断结果为金标准,分析二维超声、彩超单一及联合检测对甲状腺乳头状癌诊断价值。结果共检出71个结节,其中23例(27个结节)为恶性病变,37例(44个结节)良性病变。二维超声检查敏感度为74.07%、特异度为75.00%、约登指数为0.491;彩超检查敏感度为81.48%、特异度84.09%、约登指数为0.656;联合诊断敏感度为92.59%、特异度93.18%、约登指数0.858。联合超声诊断效能明显高于单独二维超声、彩超检查,差异有统计学意义(P<0.05)。良性与恶性结节声像图特征对比,差异有统计学意义(P<0.05)。结论超声技术应用甲状腺乳头状癌检查中,其声像图特征具备明显特异性,可为临床早期筛查提供准确依据;单独二维超声、彩超诊断效能相对较低,且单独检查中仍然存在较高的假阳性、假阴性率,联合两种技术可提高诊断效能与准确率。
Objective To explore the value of two-dimensional and color Doppler ultrasonography(color Doppler ultrasound)in the early diagnosis of papillary thyroid carcinoma.Methods 60 patients with thyroid lesions admitted to the Ultrasound Medical Department of Suining County People's Hospital from January 2020 to October 2022 were selected,all of whom underwent two-dimensional ultrasound and color ultrasound examinations.The pathological diagnosis results were used as the gold standard to analyzed the diagnostic value of two-dimensional ultrasound,color ultrasound single and combined detection for papillary thyroid cancer.Results A total of 71 nodules were detected,of which 23(27 nodules)were malignant lesions and 37(44 nodules)were benign lesions.The sensitivity,specificity and Yoden index of two-dimensional ultrasound examination were 74.07%,75.00%and 0.491,respectively.The sensitivity,specificity,and Yoden index of color ultrasound examination were 81.48%,84.09%,and 0.656,respectively.The sensitivity of combined diagnosis was 92.59%,the specificity was 93.18%,and the Yoden index was 0.858.The diagnostic efficacy of combined ultrasound was significantly higher than that of individual two-dimensional ultrasound and color ultrasound examinations,the difference was statistically significant(P<0.05).Comparison of ultrasound features between benign nodules and malignant nodules,the difference was statistically significant(P<0.05).Conclusion Ultrasonic technology is used in the examination of papillary thyroid carcinoma,and its sonographic characteristics have obvious specificity,which can provide accurate basis for early clinical screening.The diagnostic efficiency of two-dimensional ultrasound and color Doppler ultrasound alone is relatively low,and there are still high false positive and false negative rates in the single examination.Combining the two techniques can improve the diagnostic efficiency and accuracy.
