摘要
目的 探讨医用振动排痰机辅助高频率叩背排痰对胸腔镜肺癌根治术后患者排痰效果及肺部并发症的影响。方法 选取2020年6月至2021年8月于浙江省台州医院住院的110例胸腔镜肺癌根治术后患者,随机分为对照组和观察组,各55例。对照组术后予以高频率叩背排痰法,观察组术后在对照组基础上加用医用振动排痰机干预,两组均干预5 d,观察两组干预前和干预5 d排痰30 min后血气分析指标的变化,比较24 h平均排痰量及并发症发生率。结果 干预5 d排痰后30 min,两组动脉血氧分压(PaO_(2))指标均高于干预前,动脉血二氧化碳分压(PaCO_(2))指标均低于干预前,且观察组PaO_(2)指标高于对照组,PaCO_(2)指标低于对照组,差异有统计学意义(P<0.05);观察组术后24 h平均排痰量多于对照组,观察组术后并发症发生率低于对照组,差异均有统计学意义(P<0.05)。结论 医用振动排痰机辅助高频率叩背排痰用于胸腔镜肺癌根治术后患者较单纯高频率叩背排痰法更具优势,不仅可增强肺通气和换气功能,改善血气分析指标水平,而且可提升排痰效果,预防并发症的发生。
Objective The influences of medical vibration expectoration machine assisted by high frequency percussion back on the expectoration effects and pulmonary complications of patients after thoracoscopic radical lung cancer surgery were investigated.Methods A total of 110 patients admitted to Taizhou Hospital of Zhejiang Province from June 2020 to August 2021 after thoracoscopic radical lung cancer surgery were selected and randomly divided into a control group and an observation group,with 55 cases in each group.The control group was treated with high frequency percussion back expectoration after surgery,and the observation group was treated with medical vibration expectoration machine after surgery on the basis of the control group.Both groups were treated for 5 days.The changes of blood gas analysis indexes of both groups were observed before intervention and 30 minutes after 5 days of expectoration,and the average expectoration volume within 24 hours and complication rate were compared.Results 30 minutes after 5 days of intervention,the arterial partial pressure of oxygen(PaO2)of the two groups were higher than before intervention,and the arterial partial pressure of carbon dioxide(PaCO2)of the two groups were lower than before intervention,and PaO2 of the observation group was higher than the control group and the PaCO2 of the observation group was lower than the control group,the differences were statistically significant(P<0.05).The average expectoration volume within 24 hours after operation in the observation group was higher than that in the control group,and the incidence of postoperative complications in the observation group was lower than that in the control group,with statistically significant differences(P<0.05).Conclusion The application of medical vibration expectoration machine assisted by high frequency percussion back expectoration in patients after thoracoscopic radical lung cancer surgery has more advantages than the simple high frequency percussion back expectoration,which can not only enhance
