摘要
目的:探讨胎儿颈后透明层厚度(NT)的"叠加线"征在中矢状面犁骨-上颌结合部二维成像上早期诊断胎儿继发性腭裂的应用。方法:使用二维超声(中矢状面)对2022年1月~2023年1月来漳浦县医院接受常规产检筛查的1990例11~14孕周的单胎胎儿叠加线征进行评估。如果存在第二腭的中线裂开的情况,则考虑不存在由腭形成的线,此时仅能看到犁骨,从而只形成单线而不是正常的叠加双线。对于无叠加线征的病例,再进行孕中期的筛查确认。结果:3例继发腭裂胎儿均在NT期被发现无叠加线,在孕中期被评估证实。这3例均为孤立性继发性腭裂,且缺损累及上颌前部。结论:叠加线征是评估胎儿继发性腭裂的一种超声标记物;该体征的记录证明了中线同时存在腭部和犁骨。该标记物可在孕早期晚期明确显示,对诊断继发性腭裂可以起到提示帮助。
Objective:To investigate the application of the"superimposed line"sign described by the nuchal translucency(NT)in the early diagnosis of fetal secondary cleft palate on the two-dimensional imaging of the vomeronasal maxillary junction in the midsagittal plane.Methods:This is a prospective evaluation of the superimposed line signs of 1990 singleton fetuses who underwent routine prenatal screening at 11 to 14 gestational weeks in Zhangpu County Hospital from January 2022 to January 2023 using two-dimensional ultrasound(midsagittal plane).In this view,the vomer bone appears as a line superimposed at the distal two-thirds of the maxillary line,because the vomer bone fuses with the second palate at the midline.If there is a midline cleft of the second palate,it is considered that there is no line formed by the palate,so only the vomer bone can be seen at this time,so that only a single line instead of a normal superimposed double line is formed.For cases without superimposed line sign,we will screen and confirm in the second trimester of pregnancy.Results:33 fetuses with secondary cleft palate were found to have no superimposed line in NT stage and were confirmed by evaluation in second trimester.All 3 cases were isolated secondary cleft palate with defects involving the anterior maxillary region.Conclusion:Overlap line sign is an ultrasonic marker to evaluate secondary cleft palate in fetus.The recording of this sign evidences the presence of both the palate and the vomer along the midline.The marker can be clearly shown in the late pregnancy,and can play a role in promoting the diagnosis of secondary cleft palate.
