摘要
目的分析和探讨通过宏基因组高通量测序(mNGS)诊断的8例发热伴血小板减少综合征(SFTS)患者的临床特点,提高对mNGS在SFTS诊断中的应用认识。方法回顾性分析2010年1月至2022年10月郑州大学第一附属医院收治的8例SFTS患者的临床资料,总结mNGS的诊断价值及该病的临床特点、实验室检查、影像学检查、治疗方法及短期预后。结果8例患者中男4例,女4例,发病年龄(52.88±9.31)岁,均为农民,3例为重型,5例为危重型,均有发热、乏力、纳差、肌肉酸痛、血小板减少、多器官功能障碍。血或肺泡灌洗液mNGS检出新型布尼亚病毒(SFTSV)序列,给予抗病毒等对症治疗后6例患者好转,2例患者死亡,该2例SFTSV序列数较高。结论SFTS患者易出现发热、乏力、肌肉酸痛、血小板减少、多器官功能障碍,mNGS对SFTS的诊断具有重要价值。
Objective To analyze the clinical characteristics of 8 cases with severe fever with thrombocytopenia syndrome(SFTS)diagnosed by metagenomics next-generation sequencing(mNGS)and to improve the knowledge of the diagnosis of mNGS in SFTS.Methods The clinical data of 8 patients with SFTS who were recruited from the First Affiliated Hospital of Zhengzhou University from January 2010 to October 2022 were retrospectively analyzed.The diagnostic value of mNGS and the clinical characteristics,laboratory tests,imaging examinations,treatment method and short-term prognosis of this disease were summarized.Results Among the 8 patients,4 were males and 4 were females,and the age of onset was(52.88±9.31)years.All of them were farmers,3 cases were severe type,5 cases were critical type.All of them complain of fever,fatigue,anorexia,muscle soreness,thrombocytopenia and multiple organ dysfunction.Severe fever with thrombocytopenia syndrome Bunya virus(SFTSV)sequence was detected in blood or alveolar lavage fluid mNGS.After symptomatic treatment such as antiviral therapy,6 patients improved and 2 patients died.The number of SFTSV sequences in these 2 patients was high.Conclusion Patients with SFTS are prone to fever,fatigue,muscle soreness,thrombocytopenia and multiple organ dysfunction.mNGS is of great significance in diagnosing SFTS.
