摘要
目的比较二尖瓣自动分析软件MVN与经胸二维超声(2D⁃TTE)、经食管实时三维超声(RT⁃3D⁃TEE)对二尖瓣脱垂(MVP)区域的诊断效能。方法选取北部战区总医院自2021年1月至2022年6月收治的接受超声心动图检查并确诊为MVP的18例患者为研究对象。患者均接受2D⁃TTE、RT⁃3D⁃TEE、MVN检查及外科手术治疗,对3种不同方法诊断的MVP区域与术中结果进行对比分析。结果在18例患者的108个二尖瓣区域中,术中见MVP区域28例。2D⁃TTE检查MVP的敏感性为78.57%(22/28),特异性为97.50%(78/80);RT⁃3D⁃TEE检查MVP的敏感性为89.29%(25/28),特异性为98.75%(79/80);MVN检查MVP的敏感性为89.29%(25/28),特异性为98.75%(79/80)。结论RT⁃3D⁃TEE能够模拟术中视野,对MVP做出明确诊断,结合MVN可以定量分析二尖瓣相关参数,对于术前制定手术计划、术中监测及术后评估等具有重要临床意义。
Objective To compare the diagnostic efficacy of automatic mitral valve analysis software MVN(Mitral Valve Navigator)with two⁃dimensional transthoracic(2D⁃TTE)and real⁃time three⁃dimensional transesophageal echocardiography(RT⁃3D⁃TEE)in the mitral valve prolapse(MVP)region.Methods Eighteen patients diagnosed with MVP who received echocardiography and were admit⁃ted to General Hospital of Northern Theater Command from January 2021 to June 2022 were selected as the study objects.All patients received 2D⁃TTE,RT⁃3D⁃TEE,MVN examination and surgical treatment.The MVP region diagnosed by three different methods was compared with the intraoperative results.Results Of the 108 mitral valve regions in 18 patients,the MVP region was seen intraopera⁃tively in 28 cases.The sensitivity and specificity of 2D⁃TTE tests for MVP were 78.57%(22/28)and 97.50%(78/80).The sensitiv⁃ity and specificity of RT⁃3D⁃TEE were 89.29%(25/28)and 98.75%(79/80).The sensitivity and specificity of MVN test for MVP were 89.29%(25/28)and 98.75%(79/80).Conclusion RT⁃3D⁃TEE can simulate intraoperative visual field and make a clear di⁃agnosis of MVP.Combined with MVN,mitral valve⁃related parameters can be quantitatively analyzed,which has important clinical sig⁃nificance for preoperative operation planning,intraoperative monitoring and postoperative evaluation.
