摘要
目的探讨高血压脑出血早期血肿扩大的相关危险因素。方法回顾性分析2014年10月至2021年10月厦门大学附属福州第二医院收治的高血压脑出血患者265例,根据是否发生血肿扩大将患者分为血肿扩大组(70例)和血肿稳定组(195例)。收集两组患者的一般资料、凝血功能指标、离子钙水平、CT影像学特征、凝血功能综合评分(coagulation functionalcompositescore,CFCS)等资料。采用多因素Logistic回归分析探讨高血压脑出血患者早期血肿扩大的危险因素。结果两组患者的年龄、入院时收缩压、CFCS评分、离子钙水平、出血征、入院时血肿量比较,差异均有统计学意义(P<0.05);多因素Logistic回归分析结果显示,CFCS评分、离子钙水平、出血征、入院时血肿量、年龄均是影响高血压脑出血患者早期血肿扩大的独立危险因素(P<0.05)。结论高CFCS评分、离子钙水平降低、出血征、入院时血肿量大、年龄均是影响高血压脑出血患者早期血肿扩大的独立危险因素,对预测高血压脑出血早期血肿扩大具有较高的临床指导价值。
Objective To investigate the risk factors of hematoma enlargement in early stage of hypertensive intracerebral hemorrhage.Methods A retrospective analysis was performed on 265 patients with hypertensive intracerebral hemorrhage admitted to Fuzhou Second Hospital Affiliated to Xiamen University from October 2014 to October 2021.The patients were divided into hematoma enlargement group(70 cases)and hematoma stabilization group(195 cases)according to whether hematoma enlargement occurred.General data,coagulation function index,ionic calcium level,CT imaging features,coagulation functional composite score(CFCS)and other data of the two groups were collected.Multivariate Logistic regression analysis was used to investigate the risk factors of early hematoma enlargement in patients with hypertensive intracerebral hemorrhage.Results There were significant differences in age,systolic blood pressure at admission,CFCS score,ionic calcium level,bleeding sign and hematoma amount at admission between the two groups(P<0.05).Multivariate Logistic regression analysis showed that CFCS score,ionic calcium level,hemorrhage sign,hematoma amount at admission,and age were independent risk factors for early hematoma enlargement in hypertensive intracerebral hemorrhage patients(P<0.05).Conclusion High CFCS score,decreased ionic calcium level,hemorrhage sign,large hematoma amount at admission,and age are all independent risk factors for early hematoma enlargement in hypertensive intracerebral hemorrhage patients,and have high clinical guiding value for predicting early hematoma enlargement.
