摘要
目的探讨提高健康体检心电图(ECG)质量控制的措施。方法回顾性分析清华大学第一附属医院健康管理中心2020年1月—2021年12月采集的所有受检者ECG的质量情况。使用心电图机(福田,型号FX-8322),采集标准12导联心电信号。2020年1月—12月按照常规标准采集心电图11639例份,受检者年龄14~92岁,男性5543例、女性6096例;2021年在分析2020年心电图采集状况的基础上提高了质量把控严格程度,2021年1月—12月采集16351例份,受检者年龄12~94岁,男性8611例、女性7740例,比较经提高心电图质量控制后心电图分析准确率。统计使用SPSS23.0软件,分类变量描述采用例数(%)表示,组间比较采用χ^(2)检验,P<0.05为差异具有统计学意义。结果2020年受检者的ECG采集质控和诊断质控不合格率分别为3.8%(442/11639)和0.3%(35/11639),2021年分别为0.4%(65/16351)和0.1%(16/16351);两者ECG采集质控不合格率(χ^(2)=441.938)和诊断质控不合格率(χ^(2)=15.384),差异均具有统计学意义(P均<0.001)。结论严格控制ECG质量,规范心电检查流程,可更好地发挥ECG在健康体检中的作用。
Objective To explore measures to improve the quality control of electrocardiogram(ECG)in health check-ups.Methods A retrospective analysis was conducted on the quality of ECG collected from all subjects at the Health Management Center of the first hospital of Tsinghua university from January 2020 to December 2021.Standard 12-lead ECG signals were collected using an ECG machine(Futian,Model FX-8322).From January to December 2020,11639 ECGs were collected according to routine standards,with the age of the subjects ranging from 14 to 92 years,including 5543 males and 6096 females.Based on the analysis of the ECG collection status in 2020,the quality control was improved in 2021.From January to December 2021,16351 ECGs were collected,with the age of the subjects ranging from 12 to 94 years,including 8611 males and 7740 females.The accuracy of ECG analysis was compared after improving the quality control of ECGs.Statistical analysis was performed using SPSS 23.0 software,with categorical variables expressed as(%),and theχ^(2) test used for inter-group comparisons.A significance level of P<0.05 was considered statistically significant.Results The unqualified rates of ECG collection and diagnostic quality control for subjects in 2020 were 3.8%(442/11639)and 0.3%(35/11639),respectively.In 2021,the unqualified rates were 0.4%(65/16351)and 0.1%(16/16351),respectively.The differences in the unqualified rates of ECG collection quality control(χ^(2)=441.938)and the diagnostic quality control(χ^(2)=15.384)were statistically significant in both years(all P<0.001).Conclusions Strictly controlling the quality of ECGs and standardizing the ECG examination process can better utilize ECGs in health check-ups.
