摘要 目的探究精益管理方法在优化创伤中心患者救治流程中的应用效果。方法本研究为回顾性研究。搜集2021年10月—2022年10月北京市门头沟区医院急诊科195例创伤患者的病例资料为观察组,2020年9月—2021年9月175例创伤患者为对照组。观察组采用精益管理优化后的创伤中心患者救治流程,对照组采用传统的急诊工作流程及管理模式,比较两组专科会诊医师到达时间、全身快速CT检查时间、抢救室滞留时间,以上指标均为符合正态分布的计量资料,以xˉ±s表示,采用t检验;比较两组抢救成功率采用χ^(2)检验。结果观察组专科会诊医师到达时间平均(5.4±1.4)min、全身快速CT检查时间平均(27.8±5.4)min、抢救室滞留时间平均(51.6±7.4)min,均明显短于对照组的(17.5±5.3)min、(44±8.2)min、(86.2±10.2)min,差异均有统计学意义(t值分别为30.71、22.66、37.61,P值均<0.05);观察组抢救成功率74.3%(145/195),明显高于对照组58.4%(102/175),差异有统计学意义(χ^(2)=10.74、P<0.05)。结论精益管理方法应用于优化创伤中心救治流程,有利于缩短创伤患者的抢救和滞留时间,提高抢救成功率。 Objective To explore the application effectiveness of lean management methods in optimizing the treatment process of patients in the trauma center.Methods This study was a retrospective study.The case data of 195 trauma patients from the Emergency Department of Beijing Mentougou District hospital,from October 2021 to October 2022,were collected as the observation group,while 175 trauma patients from September 2020 to September 2021 were used as the control group.The observation group adopted the optimized trauma center patient treatment process using lean management,while the control group followed the traditional emergency workflow and management mode.The arrival time of specialist consultant physicians,the time for whole-body rapid CT scans,and the time spent in the emergency room were compared between the two groups.These indicators were all quantitative data that conformed to a normal distribution and expressed as xˉ±s.The t-test was used for statistical analysis.The comparison of the success rate of resuscitation between the two groups and analyzed using theχ^(2)-test.Results The average arrival time of trauma specialists in the observation group was(5.4±1.4)minutes,and the average time for whole-body rapid CT scans was(27.8±5.4)minutes,and the average time spent in the emergency room was(51.6±7.4)minutes.These times were significantly shorter than those in the control group,which were(17.5±5.3)minutes,(44±8.2)minutes,and(86.2±10.2)minutes,respectively.The differences were statistically significant(t=30.71,22.66,37.61;all P<0.05);The resuscitation success rate in the observation group was 74.3%(145/195),which was significantly higher than the control group's rate of 58.4%(102/175).The difference was statistically significant(χ^(2)=10.74,P<0.05).Conclusion The application of lean management methods to optimize the treatment process in trauma centers is beneficial in reducing the rescue and stay time for trauma patients and improving the success rate of resuscitation.
