血清SP、MT、HSP70水平与口腔扁平苔藓病损程度及细胞免疫-炎症反应的关系

Relationship between serum SP,MT,HSP70 levels and the degree of lesion and cellular immune-inflammatory response in oral lichen planus

摘要 目的探讨血清P物质(SP)、褪黑素(MT)、热休克蛋白70(HSP70)与口腔扁平苔藓(OLP)患者病损程度及细胞免疫-炎症反应的关系。方法选取2020年7月至2021年7月该院收治的45例OLP患者作为OLP组,45例健康体检者作为对照组,比较OLP组和对照组血清SP、MT、HSP70水平及细胞免疫-炎症反应介质水平。将45例OLP患者根据OLP临床分型诊断标准分为糜烂型(糜烂组,n=22)、非糜烂型(非糜烂组,n=23),比较糜烂组和非糜烂组血清SP、MT、HSP70水平,并分析SP、MT、HSP70水平与细胞免疫-炎症反应的相关性。结果OLP组血清MT水平明显低于对照组,SP和HSP70水平均明显高于对照组,差异均有统计学意义(P<0.05)。OLP组血清肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6及IL-8水平均明显高于对照组,OLP组CD3^(+)、CD4^(+)、CD8^(+)水平均明显低于对照组,CD19^(+)和CD4^(+)/CD8^(+)均明显高于对照组,差异均有统计学意义(P<0.05);OLP组和对照组CD16^(+)56水平比较,差异无统计学意义(P>0.05)。糜烂组血清MT水平明显低于非糜烂组,SP和HSP70水平均明显高于非糜烂组,差异均有统计学意义(P<0.05)。SP、HSP70水平与CD3^(+)、CD4^(+)、CD8^(+)水平均呈负相关(P<0.05),与CD4^(+)/CD8^(+)、TNF-α、IL-6、IL-8水平均呈正相关(P<0.05)。MT水平与TNF-α水平呈负相关(P<0.05),与CD3^(+)、CD4^(+)、CD8^(+)水平均呈正相关(P<0.05)。结论OLP患者血清SP、MT、HSP70水平均呈异常表达,随着病损程度加重,SP和HSP70水平升高,MT水平明显降低,且可能共同参与调节患者的细胞免疫-炎症反应过程,检测血清SP、MT、HSP70水平有益于OLP患者的病情评估及诊治方案选择。 Objective To investigate the relationship between serum Substance P(SP),melatonin(MT),heat shock protein 70(HSP70)levels and the degree of lesion and cellular immune-inflammatory response in oral lichen planus(OLP).Methods A total of 45 OLP patients admitted to the hospital from July 2020 to July 2021 were selected as the OLP group,and 45 healthy subjects were selected as the control group.The serum levels of SP,MT,HSP70 and the levels of cellular immune-inflammatory response mediators were compared between the OLP group and the control group.Base of 45 cases of OLP patients according to the diagnostic criteria for clinical classification of OLP into erosion type(erosion group,n=22),the erosion type(non-erosion group,n=23).The serum levels of SP,MT and HSP70 levels were compared between erosion group and non-erosion group,and the correlation among SP,MT,HSP70 levels and cellular immune-inflammatory response was analyzed.Results The serum MT level in OLP group was significantly lower than that in control group,and the serum levels of SP and HSP70 were significantly higher than those in control group(P<0.05).The serum levels of tumor necrosis factor-α(TNF-α),interleukin(IL)-6 and IL-8 in OLP group were significantly higher than those in control group.The levels of CD3^(+),CD4^(+),and CD8^(+)in OLP group were significantly lower than those in control group,and CD19^(+)and CD4^(+)/CD8^(+)were significantly higher than those in control group(P<0.05).There was no significant difference in the level of CD16^(+)56 between OLP group and control group(P>0.05).The serum MT level in erosion group was significantly lower than that in non-erosion group,and the levels of SP and HSP70 were significantly higher than those in non-erosion group,and the differences were statistically significant(P<0.05).The levels of SP and HSP70 were negatively correlated with the levels of CD3^(+),CD4^(+)and CD8^(+)(P<0.05),and positively correlated with the levels of CD4^(+)/CD8^(+),TNF-α,IL-6 and IL-8(P<0.05).The level of MT was negatively co
