摘要
目的分析2型糖尿病(type 2 diabetes mellitus,T2DM)初发患者血管内皮功能与颈动脉病变的关系。方法回顾性分析2020年8月—2022年7月菏泽市牡丹人民医院体检中心的68例初发T2DM患者的临床资料,将其作为研究组;选取同期于本院进行体检的70名健康对象的临床资料,将其作为对照组。两组受检者均采用彩色多普勒超声对颈动脉形态及血流动力学参数进行检测,并测量肱动脉内径变化。比较两组颈动脉IMT值、血流动力学指标[收缩期峰值流速(PSV)、舒张早期反向血流峰值速度(PRV)、动脉搏动指数(PI)、舒张末期血流速度(EDV)]、肱动脉内径[基础内径、内皮依赖性血管扩张功能(FMD)、非内皮依赖性血管扩张功能(NMD)],并分析T2DM患者颈动脉IMT值与肱动脉内皮功能指标的相关性。结果研究组颈动脉IMT值高于对照组,差异有统计学意义(P<0.05)。研究组EDV、PSV、PRV、PI指标水平均低于对照组,差异有统计学意义(P<0.05)。两组肱动脉基础内径、NMD指标比较,差异无统计学意义(P>0.05)。研究组FMD指标水平低于对照组,差异有统计学意义(P<0.05)。经Pearson相关性分析显示,颈动脉IMT值与肱动脉FMD呈负相关性(P<0.05),与肱动脉NMD无相关性(P>0.05)。结论T2DM初发患者血管内皮功能与颈动脉病变关系密切,超声可以准确且无创地评估患者血管内皮功能与颈动脉病情变化。
Objective To analyze the relationship between vascular endothelial function and carotid artery lesions in patients with type 2 diabetes mellitus(T2DM)with initial onset.Methods Retrospective analysis of the clinical data of 68 newly diagnosed T2DM patients from the physical examination center of Mudan People's Hospital in Heze City from August 2020 to July 2022,and they were used as the study group;at the same time,the clinical data of 70 healthy subjects who underwent physical examinations in the hospital during the same period were selected,and they were used as a control group.Color Doppler ultrasound was used to detect the carotid artery morphology and hemodynamic parameters,and to measure the changes in the diameter of the brachial artery in both groups.The carotid IMT value,hemodynamic indexes[peak systolic velocity(PSV),early diastolic peak reverse velocity(PRV),arterial pulsatility index(PI),end diastolic velocity(EDV)],brachial artery diameter[basic diameter,endothelium-dependent vasodilation(FMD),endothelium-independent vasodilation(NMD)]were compared between the two groups,the correlation between carotid IMT and brachial artery endothelial function in T2DM patients was analyzed.Results The IMT value of the carotid artery in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).The levels of EDV,PSV,PRV,and PI indicators in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference between the two groups in the basic diameter of brachial artery and NMD index(P>0.05).The level of FMD indicators in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that carotid IMT value was negatively correlated with brachial artery FMD(P<0.05),but not with brachial artery NMD(P>0.05).Conclusion Vascular endothelial function is closely related to carotid arte
