摘要目的验证加速康复外科理念(enhanced recovery after surgery,ERAS)结合家属心理干预在腹股沟斜疝患儿围术期护理中的应用效果。方法选择2021年1月—2022年6月本院收治的行腹股沟斜疝手术治疗的患儿100例作为研究对象,随机数表法分为对照组和实验组两组,每组各50例。对照组采用常规外科护理;实验组采用ERAS结合患儿家属心理干预,即为患儿提供ERAS围术期护理方案,并为患儿母亲提供焦虑情绪的心理干预。比较两组患儿术后疼痛、并发症、住院时间等指标以及患儿母亲的焦虑情绪和护理满意度。结果实验组患儿在术后4小时和24小时疼痛评分均低于对照组,术后躁动、恶心呕吐、切口渗血发生例数均低于对照组,差异均有统计学意义(P<0.05)。实验组较对照组住院天数和住院费用均有所减少(P<0.05)。实验组患儿母亲的焦虑情绪评分在心理干预后低于对照组,护理满意度高于对照组,差异均有统计学意义(P<0.05)。结论基于ERAS结合患儿家属心理干预的护理模式可明显降低患儿术后疼痛与并发症,促进康复,缩短住院时间,降低住院费用;并有效缓解患儿母亲的焦虑情绪,提高护理满意度。 Objective To verify the application effect of enhanced recovery after surgery(ERAS) combined with psychological intervention among family members in perioperative nursing of children with indirect inguinal hernia.Methods 100 children with indirect inguinal hernia who received operation in our hospital from January 2021 to June 2022 were enrolled as study subjects,and were randomly divided into control group and experimental group according to random number table method,with 50 cases in each group.The control group received routine surgical nursing.The experimental group adopted ERAS combined with psychological intervention on family memberschildren,that is,to provide children with ERAS perioperative nursing program,and provide psychological intervention on anxiety for children's mothers.The indexes of postoperative pain,complications,hospital stay,anxiety and nursing satisfaction of the mothers between the two groups were compared.Results The pain scores of childrenat 4 hours and 24 hours after operation in the test group were lower than those in the control group,and the number of restlessness,nausea and vomiting,and incision bleeding were lower than those in the control group,with significant differences(P<0.05).Compared with the control group,the number of hospital stay and the cost of hospitalization in the test group decreased(P<0.05).The anxiety scores of mothers in the test group were lower than those in the control group after psychological intervention,and the nursing satisfaction was higher than that in the control group,with significant differences(P<0.05).Conclusions The nursing model based on ERAS combined with psychological intervention of children's family members can significantly reduce postoperative pain and complications,promote rehabilitation,shorten hospital stay and reduce hospital costs.It can effectively relieve the anxiety of the mothers of the children and improve the satisfaction of nursing.
