摘要
目的:观察针刺联合高低频重复经颅磁刺激(rTMS)对脑卒中患者上肢运动功能障碍的疗效。方法:将60例脑卒中患者随机分为对照组与研究组。两组均给予常规药物治疗、康复治疗及体针治疗。研究组采用病灶侧高频、健侧低频的rTMS治疗模式,同时根据r TMS定位的病灶侧M1区进行头针治疗;对照组采用假头针治疗。治疗前后采用上肢运动功能评定量表(FMA-UE)、中国香港版偏瘫上肢功能测试量表(FTHUE-HK)及Barthel指数评定量表(BI)进行评定。结果:与治疗前相比,两组患者治疗后FMA-UE量表中手评分、总分、FTHUE-HK、BI评分及研究组FMA-UE量表中腕评分均显著增高,差异具有显著性意义(P<0.01)。与对照组相比,研究组在FMA-UE量表中腕评分、手评分以及FTHUE-HK中明显增高,差异具有显著性意义(P<0.05)。结论:针刺联合高低频rTMS治疗可改善脑卒中患者上肢运动功能,尤其在改善手功能及腕功能方面疗效明显。
Objective:To investigate the effect of acupuncture combined with repetitive transcranial magnetic stimulation(rTMS)on upper limbs motor function in stroke patients.Method:Sixty patients were assigned to a control group and a treatment group randomly.Both groups received conventional medical treatment,stroke rehabilitation and body acupuncture.In addition,control group received traditional scalp acupuncture.Treatment group received high frequency rTMS over the area of the affected hemisphere in the morning,low frequency rTMS over the area of the unaffected hemisphere in the afternoon.Meanwhile,the M1 area of affected hemisphere was given scalp acupuncture according to rTMS location.The FMA-UE,FTHUE-HK and BI were used to evaluate the hand function and the activities of daily living before or after the treatment.Result:FMA-UE total scores,FMA scores of hand,FTHUE-HK and BI scores in both groups and FMA scores of wrist in the treatment group were higher than those before treatment,and the differences were statistically significant(P<0.01).Compared with the control group,the treatment group had significantly higher FMA score in wrist and hand and FTHUE-HK score,and the differences were statistically significant(P<0.01).Conclusion:Acupuncture combined with rTMS can improve the upper limbs motor function in stroke patients,especially the hand and wrist function.
