摘要
目的分析瘢痕子宫对剖宫产术中出血量的影响,探究其出血原因,并提出相应预防对策。方法回顾性分析我院产科2021年12月~2022年12月间收治的81例瘢痕子宫再次剖宫产产妇临床资料,纳为观察组,选择同期80例首次剖宫产产妇纳为对照组。比较两组产妇手术时间、术中出血量及出血率、术后2h阴道出血量、住院时间,并分析瘢痕子宫再次剖宫产出血的原因及预防措施。结果观察组剖宫产术中出血量及术中出血率、剖宫产手术开始至胎儿娩出时间、胎儿娩出至术毕时间及剖宫产总时间、术后2h出血量、术后住院时间均高于对照组(P<0.05);两组剖宫产术中出血原因比较无统计学差异(P>0.05);与未出血者比较,出血者距上次剖宫产时间更短(P<0.05),两组生产次数无显著性差异(P>0.05)。结论瘢痕子宫本身将增大产妇剖宫产难度及术中出血风险,可通过术前仔细评估产妇胎盘状态、做好手术准备、提高手术技巧,并建议产妇选择合适的再次妊娠时间来减少出血。
Objective To analyze the effect of scar uterus on the hemorrhage volume during cesarean section,explore the causes of hemorrhage and put forward the corresponding preventive measures.Methods The clinical data of 81 puerperae with re-cesarean section with scar uterus admitted to department of obstetrics of the hospital between December 2021 and December 2022 were retrospectively analyzed and the puerperae were included in observation group,and 80 puerperae with first cesarean section during the same time period were enrolled as control group.The operative time,intraoperative hemorrhage volume and hemorrhage rate,vaginal hemorrhage volume at 2 hours after operation and hospital stay were compared between the two groups,and the causes and preventive measures of hemorrhage in re-cesarean section with scar uterus were analyzed.Results The intraoperative hemorrhage volume and intraoperative hemorrhage rate during cesarean section,time from start of cesarean section to fetus delivery,time from fetus delivery to the end of operation and the total duration of cesarean section,hemorrhage volume at 2 hours after operation and postoperative hospital stay in observation group were higher or longer than those in control group(P<0.05).There were no statistical differences in the causes of hemorrhage during cesarean section between the two groups(P>0.05).Compared with puerperae without hemorrhage,the time from last cesarean section was shorter in puerperae with hemorrhage(P<0.05),and there was no significant difference in the parity between the two groups(P>0.05).Conclusion The scar uterus itself will increase the difficulty of maternal cesarean section and the risk of intraoperative hemorrhage,and evaluating the maternal placental status,preparing for operation,improving operative skills and recommending that the women choose the appropriate re-pregnancy time can reduce the hemorrhage.
