摘要
目的探讨经腹部超声与经阴道超声检查在卵巢囊肿患者中的诊断价值。方法选取2020年1月至2022年6月南昌市第五医院收治的70例卵巢囊肿患者,所有患者均行经腹部超声检查及经阴道超声检查,以病理检查结果为金标准,分析经腹部超声检查与经阴道超声检查对卵巢囊肿的诊断效能,并评估与病理检查结果的一致性。结果经病理检查证实,70例卵巢囊肿患者中,良性病变67例,包括盆腔炎性囊肿8例,卵巢冠囊肿18例,卵巢囊肿蒂扭转10例,巧克力囊肿24例,卵巢黄体囊肿7例;恶性病变3例,均为浆液性囊腺癌。经阴道超声检查诊断卵巢囊肿的准确度、特异度均高于经腹部超声检查,差异有统计学意义(P<0.05);经腹部超声检查与经阴道超声诊断卵巢囊肿的灵敏度、阳性预测值、阴性预测值相比,差异无统计学意义(P>0.05);经阴道超声检查诊断卵巢囊肿结果与病理检查结果的一致性极好(Kappa=0.775,P<0.05);经腹部超声检查诊断卵巢囊肿结果与病理检查结果的一致性差(Kappa=0.346,P<0.05)。结论在卵巢囊肿的临床诊断中使用经阴道超声检查的诊断准确度、特异度均高于经腹部超声检查,且与病理检查结果一致性极好,可为临床提供更多有价值的参考信息。
Objective The diagnostic value of transabdominal ultrasonography and transvaginal ultrasonography in patients with ovarian cysts was investigated.Methods With selection of 70 patients with ovarian cysts admitted to the Fifth Hospital of Nanchang from January 2020 to June 2022,all patients underwent transabdominal ultrasonography and transvaginal ultrasonography.With pathological examination results as the gold standard,the diagnostic efficacy of transabdominal ultrasonography and transvaginal ultrasonography in ovarian cysts was analyzed,and the consistency between the ultrasonography result and the pathological examination result was evaluated.Results Pathological examination confirmed that among 70 patients with ovarian cysts,67 case were benign lesions,8 case were pelvic inflammatory cysts,18 case were ovarian coronal cysts,10 case were ovarian cyst pedicle torsion,24 case were chocolate cysts,and 7 case were ovarian corpus luteum cysts;There were 3 cases of malignant lesions,all of which were serous cystadenocarcinoma.The accuracy and specificity of transvaginal ultrasonography in diagnosing ovarian cysts were higher than those of transabdominal ultrasonography,and the differences were statistically significant(P<0.05);There were no statistically significant differences in sensitivity,positive predictive value,and negative predictive value between transabdominal ultrasonography and transvaginal ultrasonography in diagnosing ovarian cysts(P>0.05);The consistency between the results of transvaginal ultrasonography in diagnosis of ovarian cysts and pathological examination was excellent(Kappa=0.775,P<0.05);The consistency between the results of transabdominal ultrasonography diagnosis of ovarian cysts and pathological examination was poor(Kappa=0.346,P<0.05).Conclusion The diagnostic accuracy and specificity of transvaginal ultrasonography in the clinical diagnosis of ovarian cysts are higher than those of transabdominal ultrasonography,and the consistency with pathological examination results is excellent,which
