一起人H3N8亚型流感病例相关的禽间紧急流行病学调查

摘要 2022年4月26日国家卫生健康委通报,河南省上蔡县发现一例人感染禽源H3N8亚型流感病毒病例。为探究感染病原来源,了解禽间H3N8亚型流感病毒感染情况,对该病例所在村庄及关联场点开展了禽间H3N8亚型禽流感紧急流行病学调查。在关联场点“诚信鸡店”的禽口咽-泄殖腔拭子中检出H3N8亚型流感病毒核酸阳性,推测该病例的感染可能与密切接触携带病毒的家禽相关;“诚信鸡店”售卖的禽只来自驻马店市驿城区“众信市场”,而该市场活禽大多来自外省份,因此病原通过农贸市场由外地传入的可能性较大,但不排除通过野禽传入的可能。本地家禽中未检出H3N8亚型流感病毒核酸阳性,说明病毒在本地禽群中流行的风险较低。调查提示,禽流感病毒在当地活禽交易市场存在一定的污染,且不同亚型病原共存,需进一步加强流行病学调查、监测和禽群免疫工作,通过积极宣传引导,提升养殖场户的生物安全水平。 On April 26,2022,a human case of avian H3N8 influenza was found in Shangcai County,Henan Province as announced by National Health Commission.In order to investigate the source of infection,and to find out its status in poultry,an emergency epidemiological investigation was carried out in the village where the case was found and its related sites.Positive nucleic acids of H3N8 avian influenza virus(AIV)were detected from avian oropharyngeal-cloacal swabs in Chengxin Chicken Store where the case was related,indicating that the case might be infected through directly contacting with poultry carrying with virus;the poultry sold by the store were purchased from Zhongxin Market in Yicheng District,Zhumadian City where live poultry were introduced from other provinces,so the pathogen was more likely to be introduced from other regions through farmers'markets,but possibly through wild birds.No positive nucleic acid of H3N8 avian influenza virus was detected in local poultry,indicating a low risk of spreading in the poultry population.The investigation results revealed that local live poultry markets were contaminated by AlV to certain extent,and different subtypes were compatible with each other,indicating that epidemiological investigation,monitoring and poultry immunization should be further strengthened,and the biosafety level of livestock farms/households should be improved through active publicity and guidance.
