经肛单通路磁吻合环治疗直肠狭窄的动物实验研究 被引量:1

Animal experimental study of single-channel transanal magnetic anastomosis ring in the treatment of rectal stenosis

摘要 目的设计适用于经肛单通路下治疗直肠狭窄的磁吻合环并验证其可行性。方法根据直肠狭窄解剖特点,设计既能满足经肛单通路置入又能建立较大吻合口的磁吻合环。6只新西兰兔采用开腹后丝线部分结扎法建立直肠狭窄模型,利用自行设计的经肛单通路磁吻合环完成直肠狭窄磁力再通,并评价吻合再通效果。结果经肛单通路磁吻合环由2个半圆环构成,经肛置入时成S形,进入狭窄直肠近端后变成圆形。在6只直肠狭窄实验兔模型上成功将经肛单通路磁吻合环置入狭窄直肠两端,X射线下可见经肛单通路磁吻合环自动对位相吸良好;直肠狭窄磁吻合操作时间平均(10.67±0.93)min,磁环置入后3~5 d磁吻合环经肛门自行排出;观察大体标本可见直肠吻合口形成良好。结论经肛单通路磁吻合环设计合理,操作简单,吻合效果确切,可用于经肛通路下直肠狭窄再通吻合,有望成为临床治疗直肠狭窄的可靠方法。 Objective To design a magnetic magnamosis ring for the treatment of rectal stenosis through transanal single access and verify its feasibility.Methods According to the anatomical characteristics of rectal stenosis,a magnamosis ring was designed,which can both meet the requirements of transanal single access insertion and establish a larger anastomosis.Six New Zealand rabbits were used to establish rectal stenosis model by partial silk ligation of silk thread after laparotomy.The magnetic recanalization of rectal stenosis was completed by the self-designed single-channel transanal magnetic anastomosis ring,and the anastomotic recanalization effect was evaluated.Results The single-channel transanal magnetic anastomosis ring was composed of two half rings,which formed an S shape when inserted through the anus and became an round shape after entering the narrow proximal rectum.The single-channel transanal magnetic anastomosis rings were successfully inserted into both ends of the rectum in six rabbit models of rectal stenosis.X-ray showed that the single-channel transanal magnetic anastomosis rings had good automatic alignment and attraction.The average operation time of rectal stenosis magnetic anastomosis rings was(10.67±0.93)minutes,and the magnetic anastomosis ring was discharged automatically through the anus 3 to 5 days after insertion.The gross specimen showed that the rectal anastomosis was well formed.Conclusion The single-channel transanal magnetic anastomosis ring has the advantages of ingenious design,simple operation and exact anastomosis effect,which can be used for the recanalization and anastomosis of rectal stenosis through anal canal,and is expected to become a reliable method for clinical treatment of rectal stenosis.
